Objective: Spontaneous abortion is a pregnant woman whose pregnancy is less than28 weeks and the fetal weight is less than 1kg.As one of the common gynecologic diseases,the incidence of the pregnancy is about 15% of the total pregnancy.At present,the cause of spontaneous abortion is not very clear in clinical practice.Exploring the cause and mechanism of spontaneous abortion is an important topic in clinical research.In recent years,the study found that the cause of early pregnancy abortion is very complex,in addition to genetic factors,infection,endocrine,abnormal reproductive organs and other causes,there are still nearly half of the spontaneous abortion cannot be identified.Among them,pathogen infection is one of the important factors of spontaneous abortion.It is reported that the main known pathogen infection,such as herpes simplex,mycoplasma,Toxoplasma and cytomegalovirus,is related to abortion.Human cytomegalovirus(HCMV)can cause chronic infection of the body,mostly latent infection.HCMV is the main virus causing intrauterine infection.More and more studies have shown that the rate of HCMV infection in pregnant women with spontaneous abortion is significantly higher than that of normal pregnant women.HCMV infection is one of the important causes of bad pregnancy outcome.However,the specific mechanisms and pathways of intrauterine transmission and spontaneous abortion in HCMV are not clear.Some studies have shown that the immune response can activate HCMV in peripheral blood mononuclear cells and induce autoimmune diseases.Therefore,we speculate that there may be immune mediated pathogenesis in HCMV infection.In this study,the mechanism of the relationship between human cytomegalovirus active infection and spontaneous abortion was studied through the spontaneous abortion of pregnant women in our hospital and normal pregnant women in the same period.Methods: 62 cases of spontaneous abortion pregnant women admitted to our hospital were selected as the disease group,and 51 cases of normal pregnant women were selected as the control group.The fasting peripheral blood of two groups of pregnant women was collected and HCMV-DNA was detected by real-time fluorescence quantitative PCR.The serum levels of c MV-Ig M and inflammatory cytokines(TNF-?,IL-1,IL-6)were detected by ELISA.)The secretion level.Comparing the positive rates of HCMV-DNA and c MV-Ig M in pregnant and control pregnant women,and dividing the spontaneous abortion pregnant women into active HCMV infection group and inactive infection group according to the test results,comparing the levels of inflammatory cytokine secretion To investigate the relationship between active HCMV infection and spontaneous abortion during pregnancy and possible molecular mechanisms.Results:(1)The positive rate of HCMV-DNA in the diseased group(16.13%)was higher than that in the control group(3.92%),and the positive rate of c MV-Ig M(37.10%)was also significantly higher than that in the control group(7.84%).Significant(P < 0.05).(2)The levels of serum TNF-?,IL-1 and IL-6 in the diseased group were significantly higher than those in the control group(P<0.05).(3)According to the test results,the abortion pregnant women were divided into HCMV active infection group and inactive infection group,and the differences of inflammatory cytokines TNF-?,IL-1 and IL-6 levels were compared between the two groups.The levels of serum TNF-?,IL-1 and IL-6 in the infected group were significantly higher than those in the inactive infection group(P<0.05).(4)Logistic regression analysis of serum TNF-?,IL-1,IL-6 levels and HCMV active infection in two groups of pregnant women,maximum likelihood estimation of serum TNF-?,IL-1 and IL-6 levels Both the value and the OR value were >0,indicating that serum TNF-?,IL-1,IL-6 levels were positively correlated with HCMV active infection.The higher the serum TNF-?,IL-1,IL-6 levels,the more likely to occur. Liver abortion is also more likely to occur with active HCMV infection.Conclusion: Spontaneous abortion is associated with active cytomegalovirus infection in pregnant women.Cytomegalovirus infection is one of the important causes of spontaneous abortion.HCMV infection will further induce immunosuppression in pregnant women,promote fetal intrauterine infection and may eventually lead to miscarriage.
