BackgroundSevere pneumonia has become one of the main diseases in the intensive care unit.The incidence is high and there is a growing trend.Severe pneumonia can lead to multiple inflammatory reactions in the body,and the disease develops rapidly,can gradually develop into acute respiratory distress syndrome,septic shock,and even multi-organ failure,leading to clinical death and poor prognosis.Conventional treatments such as anti-infection,phlegm resolution,protection of important organs,and nutritional support did not significantly improve the clinical prognosis.In recent years,people gradually have excessive inflammatory response to severe pneumonia,pathophysiology,and a variety of clinical studies have shown that Xuebijing injection will be a net injection in the treatment of severe pneumonia which has obvious auxiliary function,it can restrain excessive inflammatory reaction and resist oxidative stress,improve the function of blood coagulation function and microcirculation,regulate immune status.PurposeBy collecting the cases of severe pneumonia patients,the clinical data of 72 cases of severe pneumonia patients meeting the inclusion criteria ofsevere pneumonia were studied and analyzed,to explore the clinical value of Xuebijing injection in the adjuvant treatment of severe pneumonia,and to provide a basis for the diagnosis and treatment of severe pneumonia in various aspects.MethodsA retrospective analysis was conducted on 72 patients who met the inclusion criteria for severe pneumonia admitted to the intensive care department of China-Japan friendship hospital of Jilin university from September 2016 to December 2018.Clinical data were collected,and 72 patients with severe pneumonia were divided into 24 patients in the control group,20 patients in the low-dose group,and 28 patients in the high-dose group according to the use of Xuebijing injection and different doses of Xuebijing injection.Among them,the control group was treated with routine treatment including anti-infection,phlegm resolution,mechanical ventilation and nutritional support.In the low-dose group,50 ml of Xuebijing injection was added on the basis of the control group,twice a day for one week.In the high-dose group,100 ml of Xuebijing injection was added to the control group twice a day for one week.Detailed records of patients before and after 1 week treatment temperature(T),white blood cell count(WBC),calcitonin(PCT),platelet count(PLT),prothrombin time(PT),activated partial clotting enzymelive time(APTT),thrombin time(TT),fibrinogen(Fib),D-dimer(D-D),acute physiology and chronic health evaluation(APACHE ? score)index,oxygenation index(Pa O2 / Fi O2)related changes,and statistics of the ICU patients with 28 days hospitalization time,mechanical ventilation time and mortality.The results1.This retrospective study included 72 cases of severe pneumonia patients,24 cases of control group,low-dose group 20 cases,large dose group of 28 cases,by the statistical methods to analyze,in three groups of patients age,gender,and into the ICU APACHE ? score no statistical differences.2.Three groups of patients in the ICU T,the WBC,PCT,PLT,PT,APTT,TT,Fib,D-D,Pa O2 / Fi O2 to compare differences had no statistical significance,1 week after treatment T of three groups of patients,the WBC,PCT,PT,APTT,TT,D-D,APACHE ? score was decreased,the PLT and Fib,Pa O2 / Fi O2 higher than before treatment,and treated with small dose group and high-dose group 1 weeks after the change is more obvious.3.Comparison of data of the same period between the low-dose group and the high-dose group after 1 week of treatment: patients in the high-dose group showed a more significant decrease in T,WBC and PCTin the low-dose group,and a more significant increase in Pa O2/Fi O2 in the low-dose group.However,PLT,PT,APTT,TT,Fib,d-d and APACHEII scores of the two groups showed no significant difference,with no statistical significance.4.Compared with the control group,the duration of mechanical ventilation and the length of stay in the ICU in the low-dose group and the high-dose group were shorter than those in the control group.The duration of mechanical ventilation in the high-dose group was shorter than that in the low-dose group during the same period,while there was no significant difference in the length of stay in the ICU between the high-dose group and the low-dose group.5.The 28-day mortality of the control group,the low-dose group and the high-dose group was 41.7%,30% and 35.7%,respectively.There was no significant difference among the groups.ConclusionOn the basis of conventional treatment,Xuebijing injection has a certain effect on the adjuvant treatment of severe pneumonia.Blood will net injection can inhibit inflammatory reaction,improve the function of blood coagulation,reduce the APACHE ? score,improve oxygenation index,shorten the time of mechanical ventilation and ICU length of hospital stay.However,Xuebijing injection had no significanteffect on the 28-day mortality of patients with severe pneumonia and could not improve the prognosis.
