Objective: Sepsis is a severe clinical syndrome caused by inflammatory infection,septic shock caused by inflammation of systemic inflammatory response and(or)organ dysfunction syndrome,also known as septic shock.A large number of infection of interleukin,tumor necrosis factor and inflammatory factor synthesis and release into the blood or body fluids,vascular endothelial function injury induced vascular permeability increased dramatically,which is activated Hageman factor,start clotting,endogenous blood concentration,microvascular microthrombus,lead to micro-circulation,promote each other and influence factors serious when induced multiple organ failure.There was a positive correlation between coagulation dysfunction and severity and mortality of sepsis.Xuebijing injection can remove endotoxin,interleukin,tumor necrosis factor and other inflammatory factors,improve the efficacy of hypoxia,with blood stasis,detoxification,it also has the function of enhancing humoral immune function.This research adopts the blood will net injection in the treatment of septic shock,and monitoring of blood coagulation function in patients with sepsis and the change of inflammatory factors,and to observe the blood will be a net injection on blood coagulation function,the influence of inflammatory factor levels.Methods: All patients were carried out in accordance with the guidelines require reasonable liquid recovery,vascular active drugs and antibiotics,intensive insulin,protect the liver and kidney function,prevention emergency ulcer,nutritional support treatment,except the observation group were gaven Xuebijing injection.The levels of lactic acid,prothrombin time,activated partial thromboplastin time,fibrinogen,platelet count,TNF-?,IL-1,IL-6 were measured before and after treatment at 1d,3d,7d and 14 d.Results:1 Comparison of the two groups:The control group of 25 cases,including 11 cases of male,female 14 cases,aged 33-65 years,mean age 59.23±10.69 years old,weighing 69.29±12.25 kg,including 3 cases of hypertension,8 cases of coronary heart disease,12 cases of coronary heart disease,3 cases of cerebral infarction sequelae.The intervention group of 25 cases,there were 13 males and 12 females,aged 30-64 years,mean age 59.23±11.34 years old,weighing 72.34±12.50 kg,including 6 cases of hypertension,7 cases of coronary heart disease,10 cases of coronary heart disease,5 cases of cerebral infarction sequelae.There was no statistical difference between the two groups.2 Comparison of coagulation function between the two groups: There were no significant differences in PT,APTT between the two groups before and treatment for 1 day and 3 days.PT and APTT were significantly shorter in the observation group than the control group after 7 days and 14 days(P<0.05).3 Comparison of platelets count between the two groups: There were no significant differences in Platelet count between the two groups before and treatment for 1 day and 3 days.Platelet count were increased in the two groups after 7 days and 14 days(P < 0.05),but Platelet count were significantly increased in the observation group than the control group(P<0.05).There were significant difference between the two groups before and after treatment(F control =60.262,P < 0.05;F Intervention =119.414,P < 0.05),platelet counts were higher than before treatment in the two groups after 7,14 days(P<0.05).4 Comparison of lactic acid between the two groups: There were no significant differences in lactic acid between the two groups before treatment.Lactic acid were decreased in the two groups treatment for 1 day,3 days,7 days and 14 days,but there were no significant differences in lactic acid between the two groups.There were significant difference between the two groups before and after treatment(Fcontrol =76.973,P<0.05;F Intervention= 82.004,P<0.05).5 Comparison of TNF-?,IL-1,IL-6 between the two groups: There were no significant differences in TNF-?,IL-1,IL-6 between the two groups before and treatment for 1 day.TNF-?,IL-1 and IL-6 were decreased in the observation group than the control group after 3,7and 14 days(P<0.05),TNF-?,IL-1 and IL-6 were decreased for 7 days than 3 days(P<0.05),it were decreased for 14 days than 7 days in the observation group(P<0.05).There were significant difference before and after treatment in Intervention group(FTNF-? =104.952,P<0.05;FIL-1=437.422,P<0.05;FIL-6 =887.459,P<0.05).6 Comparison of mortality between the two groups: The mortality was obviously reduced in in the observation group than the control group,but there were no significant differences in lactic acid between the two groups.Conclusion:1 Xuebijing injection can significantly improve the coagulation function(PT,APTT shortened)and probably increase the number of platelets to some extent in the patients with septic shock patients.2 The levels of serum TNF-?,IL-1 and IL-6 are significantly decreased by Xuebijing injection.3 Xuebijing injection could not significantly reduce the mortality of patients with septic shock.
