Development And Application Of The Tool Of Professional Needs Of Care For Family Caregivers Of Stroke

ObjectivesTo develop a questionnaire Professional Needs of Care for home stroke caregivers which continuing the early study of the research group.To explore the level and influencing factors of the caregiver's professional needs and contrast with patients.MethodsThe first part: The Development of the Questionnaire Professional Needs of Care for Home Stroke Caregivers and Testing its Reliability and Validity.Based on the Family System Theory and the Family Support Model,with the reference of the relevant literatures' structure and tools at home and abroad,the first draft of the questionnaire was formed;the second stage: the test was passed through 2-round Delphi expert(15)consultation and a small sample(25 cases)pre-experiment,formating the test draft;next stage: the first collection of 240 valid questionnaires and the second collection of 30 questionnaires using SPSS 21.0 for project analysis,reliability test,content validity and exploratory factor analysis.The data from the 350 valid questionnaires collected for the third time was calculated using AMOS 17.0 for confirmatory factor analysis.The second part: Exploring the Level and Influencing Factors of the Professional Needs of Care for Home Stroke Caregivers.Using the general information of patients and caregivers,the Professional Needs of Care Questionnaire for Home Stroke Caregivers was a tool for the third 350 cases.Descriptive statistics,one-way ANOVA,and multiple linear regression analysis were performed using SPSS 21.0 to explore the level and influencing factors of caregiver's professional needs of care.ResultsThe first part: Forming a Formal Questionnaire.The total Cronbach's ? coefficient of the questionnaire is 0.919,the Cronbach's ? coefficient of the four factors are 0.907,0.923,0.951,0.815;the total folding coefficient is 0.819,The half-width coefficients of each factor are 0.897,0.916,0.938,0.745;the overall test-retest reliability is 0.914,and the test-retest reliability of each factor are 0.849,0.980,0.925,0.902;the combined reliability of each factor are 0.907,0.923,0.951,0.815.The mean variance of each factor are 0.681,0.785,0.592,0.606,the total content validity index S-CVI is 0.98,and the content validity index I-CVI of each item is 0.83~1.00;the result of confirmatory factor analysis: The chi-square/degree of freedom(?2/df)is 1.285,the root mean square residual(RMSEA)is 0.029,the goodness of fit index(GFI)is 0.916,and the comparison fit index(CFI)is 0.981.The index(NFI)is 0.920,all of them are within acceptable limits.The second part: the score of stroke professional knowledge is 38.09±6.91 and the average score for each entry is 4.23;the score of complications nursing needs is 39.77±11.18 and the average score for each entry is 3.62;the score of psychological support needs is 25.18±5.05 and the average score for each entry is 4.20;the score of cognitive exercise demand is 12.14±3.31 and the average score for each entry is 4.05.The average score in the psychological support dimension is higher than that complications nursing and cognitive training.The results of the univariate analysis showed: caregiver-patient relationship,marital status,education level,daily care time,family monthly income,and patient's education level,there was a statistically significant difference in the scores of BI,chronic disease,chronic pain,and fall(P<0.05).The results of multivariate analysis showed that the caregivers' daily care time,chronic disease types,and chronic pain were the main influencing factors of caregiver care needs(P < 0.05).ConclusionsThe questionnaire of Professional Needs of Care was constructed and it had good reliability and validity and satisfactory psychological measurement indicators were established.The Professional Needs of Care Questionnaire confirms that the questionnaire can be used to assess the professional needs of care for home stroke caregivers.The daily care time of the caregiver,the type of chronic disease,and chronic pain are the main influencing factors of caregiver's care needs,suggesting that intervention of caregiver's professional needs of care can be considered from these aspects,and maximizing through the development of personalized and targeted interventions.
