Path Analysis Of The High Hospitalization Expenses Of General Hospitals In A City Of Guangxi In 2016

Objective:Through the description and analysis of the hospitalization expenses in general hospitals in Guangxi in 2016,compare the distribution of hospitalization expenses between the high expenses group and the non-high expenses group,and analyze the composition of the hospitalization expenses.Find out the effect value of the influencing factors acting on the hospitalization expenses in a quantitative way,and explain the ways in which the influencing factors affect the hospitalization expenses.As well as its effect on the hospitalization cost,the relationship between the influencing factors and the hospitalization cost is explained more comprehensively.It provides a theoretical basis for the relevant health departments to put forward measures to control hospitalization expenses,reduce the incidence of high hospitalization expenses and optimize the allocation of health resources.Methods:Microsoft Excel 2013 was used for data merging,and SPSS Statistics 17.0 software was used for statistical analysis.The hospitalization days,hospitalization expenses are skewness distribution data,so use the number of cases and its percentage[N(%)]to describe the enumeration data,and the median(M)is used to describe the concentration trend of the measurement data.The differences between groups were analyzed by nonparametric Mann-Whitney test or nonparametric Mann-Whitney test.The path analysis method was used to analyze the influencing factors of high hospitalization expenses,and the main factors affecting the occurrence of high hospitalization expenses were found out.The path relationship between each influencing factor and hospitalization expenses was visually expressed in path graph drew by Amos Graph 24.0 software.The visual comparison of the constituent ratio of different types of hospitalization expenses is carried out by drawing a percentage chart by graphpad prism5.0.Results:Among all the patients,there were more male patients than female patients,and the age was mainly distributed in the middle and old group.The number of high hospitalization expenses patients accounted for 5.5%of the total hospitalization expenses patients,and the expenses accounted for 30.5%of the total hospitalization expenses in the whole year,which lead to occupied more health resources.Among the components of hospitalization expenses,the proportion of drug expenses was the highest(36.6%),and the cost of medical integrated services was relatively low(19.4%).The main type of medical insurance for inpatients is new agricultural cooperation(NCMS).The longer the hospitalization stay,the higher the hospitalization cost.The hospitalization cost level of the tertiary hospital is higher than that of the secondary hospital.The main influencing factors and their effect of circulatory system disease and cerebrovascular disease hospitalization cost are the number of hospitalization days,whether or not to operate,hospital level.The main influencing factors and their effect of hospitalization cost of circulatory system disease and cerebrovascular disease are the number of hospitalization days,whether or not to operate,hospital level.Conclusion:The unreasonable composition of hospitalization expenses in general hospitals in a city in Guangxi is still serious,and the proportion of drug expenses is still high.The proportion of medical comprehensive service expenses,which reflects the labor value of medical staff,needs to be improved.The main diseases are circulatory system diseases,mainly distributed in middle-aged and elderly male patients.The main reason for the high hospitalization cost of circulatory system diseases is the increase of hospitalization days,followed by surgical treatment.The incidence of high cost in tertiary hospitals is higher than that in secondary hospitals.In order to control the occurrence of high hospitalization expenses,it is the first measure to control the hospitalization days,followed by the control the proportion of drug costs and consumables costs.In addition,through perfecting and implementing the hierarchical diagnosis and treatment system,carrying out three-level preventive measures,controlling the unreasonable increase of hospitalization expenses,optimizing the allocation of health resources,and finally achieving the purpose of reducing the incidence of high hospitalization expenses.
