| Objective:To understand the constitution of hospitalization expenses for stroke patients in TCM hospitals with different demographic characteristics,different hospital grades,and different clinical pathways,with an emphasis on analyzing the cost of TCM.Using a variety of statistical methods to explore the factors that affect hospitalization costs,in order to find out the law,to explore the internal implicit relationship,to identify the main influencing factors affecting hospitalization costs.This study focuses on the impact of hospitalization on hospitalization costs,and provides a reference for controlling the excessive growth of medical expenses and reducing the economic burden of patients.Methods:The data of this study was from the database of the data center of China Academy of Chinese Medical Sciences.The data of the inpatient records of the inpatients met the following two conditions:The discharge time was from January 1,2015 to December 31,2015,and the discharge was diagnosed as Stroke(according to ICD-10,classification code is 161 and 163),a total of 71816 cases were screened out.In this study,a descriptive analysis of the data was conducted to understand the basic conditions of stroke patients and the composition of hospitalization costs.Chi-square test,rank sum test,and path analysis were used to analyze the inpatient costs and its influencing factors in hospitals for stroke patients.Results:The average cost of hospitalization for stroke patients in Chinese medicine hospitals was 7,743 yuan,and the average hospital stay was 6.6 days.A chi-square test of the patient’s hospitalization cost structure found that the cost of medicine accounted for 47.97%of the total cost of hospitalization for stroke patients,which is the most important component of the total cost of hospitalization.Univariate analysis of possible factors influencing the hospitalization costs of patients with stroke found hospital grade,regional distribution,patient gender,payment method,treatment category,clinical pathway,admissions route,dialectical nursing,Chinese medicine diagnosis and treatment equipment,Chinese medicine diagnosis and treatment technology,medical institutions Chinese medicine preparations and the implementation of the surgery have an impact on hospitalization costs.Further analysis of the path analysis found that factors such as the actual length of hospital stay,whether the operation was performed,the number of other diagnoses,the method of leaving the hospital,the number of diagnostic operations,etc.,had a greater impact on hospitalization costs.In addition to hospitalization days,other factors can not only directly affect the cost of hospitalization,but also have an indirect effect on hospitalization costs by affecting the length of stay.Conclusion:The cost of hospitalization for stroke patients in TCM hospitals is unreasonable,and over-medical phenomena such as "taking medicine to provide medicine and checking equipment for medical treatment" are serious,and hospitalization days have a great impact on hospitalization costs.The classification and treatment system should be improved and the establishment of a medical association should be accelerated to reduce the hospitalization days for patients with stroke.At the same time,strengthen prevention and health care for patients with stroke,eliminate medical supplies,optimize the adjustment of medical service prices,and reduce hospitalization costs. |
