| Objectives:By examining the dynamic changes of blood flow velocity index,coagulation function and D-dimer level in patients undergoing gynecological pelvic surgery,the effect of Xuefu Zhuyu Decoction on various indexes of the laboratory was evaluated,and the prevention of gynecology was evaluated.The efficacy of deep venous thrombosis(DVT)after pelvic surgery and the safety of its application can provide reference for the prevention of DVT after gynecological pelvic surgery.Methods:According to the inclusion criteria,exclusion criteria,rejection criteria and shedding criteria,70 patients who underwent hysterectomy in the inpatient department of The Third Hospital in Wuhan from June 2017 to December 2018 were randomly divided into experimental group:35 patients.35 cases in the control group.The control group was given intramuscular injection of low molecular weight heparin and the patient was instructed to perform lower limb functional exercise.The experimental group started taking Xuefu Zhuyu Decoction on the first day after surgery,twice a day,once 50 ml,and even served for 5 days.The clinical manifestations of the two groups were observed.Special attention was paid to the observation of the straight leg extension test(Homas sign)and the compression of the calf test(Neuhof sign).Blood flow velocity changes,prothrombin time(PT),activated partial thromboplastin time(APTT),and fiber were measured in the two groups before surgery 1 day,1 day after surgery(before administration),and 6 days after surgery.The level of proteinogen(FIB)and plasma D-dimer(D-dimer)was measured on the 6th postoperative day.The statistical analysis of the results of the two groups of laboratory indicators was carried out to observe the effect of Xuefu Zhuyu Decoction on the coagulation and fibrinolysis system and its preventive treatment of deep venous thrombosis.Results:Laboratory indicators: On the first day after operation,the changes of blood flow velocity(whole blood viscosity,plasma viscosity),fibrinogen and D-dimer were significantly higher than those before surgery(P<0.05).Statistically significant,there was no significant difference between the groups(P>0.05).On the 6th day after operation,the blood flow rate change index,fibrinogen and D-dimer value of the experimental group were lower than the control group(P<0.05),statistically significant,the whole blood high-cut and medium-viscosity,fibrinogen,D-dimer values of the observation group were basically restored to the preoperative state(P>0.05),whole blood low-cut viscosity,plasma viscosity Slightly lower than preoperative(P <0.05),statistically significant.There were no significant changes in PT and APTT levels between the two groups(P>0.05).(2)Color Doppler ultrasonography: 1 patient in the control group showed abnormal color ultrasound on the 6th day after operation.The results showed that the blood flow in the left iliac vein was slow,no obvious thrombosis was found in the color Doppler ultrasound,and the patients had appropriate lower limbs.Give Xuefu Zhuyu Decoction plus orally,and review the veins of the lower extremities after 5 days to show no obvious abnormalities.(3)All patients had no general symptoms and signs of deep venous thrombosis of the lower extremities.No adverse events such as vaginal stump bleeding or abdominal incision bleeding were observed during the oral administration of Xuefu Zhuyu Decoction.Conclusion:After hysterectomy,the blood of women is hypercoagulable.Oral administration of Xuefu Zhuyu Decoction can significantly improve the hypercoagulability of blood and regulate the activity of coagulation and fibrinolysis,thus effectively reducing the risk of postoperative deep venous thrombosis of both lower extremities.And the clinical application is safe and reliable. |
