BACKGROUND Osteoarthritis(OA)is a kind of chronic joint disease common in middle-aged and elderly people.Pathological changes mainly include degeneration of articular cartilage,synovial inflammatory hyperplasia and osteophyte formation,which can be caused by multiple factors.The knee and hip joint with heavy weight are most easily affected.With the coming of the aging society in China,the incidence of OA is increasing year by year,and the treatment of OA is becoming a major medical challenge in the future.With the deepening of OA research,people gradually realized that cytokines such as Ghrelin,IL-1? and TNF-? play a very important role in the occurrence and development of osteoarthritis.In this study,the expression of Ghrelin,IL-1? and TNF-? in the cartilage and synovial membrane of different Mankin pathological grades of knee osteoarthritis was determined by immunohistochemistry to explore the correlation of Ghrelin,IL-1? and TNF-? in the development of KOA,providing theoretical basis and feasible guidance for clinical treatment.PART ? THE EXPRESSION OF GHRELIN,IL-1? AND TNF-? IN THE PATHOLOGIC GRADE DEGENERATION CARTILAGE OF DIFFERENT MANKIN IN KNEE OSTEOARTHRITISOBJECTIVE: Immunohistochemistry was used to detect the expression of Ghrelin,IL-1? and TNF-? in the pathologic grade degeneration cartilage of different Mankin in osteoarthritis of knee,and to explore the correlation of Ghrelin,IL-1? and TNF-? in the development of KOA,so as to provide theoretical basis and feasible guidance for clinical treatment.METHODS:From 34 cases with different degree of degeneration of knee osteoarthritis cartilage specimens and 6 cases of lower limb destructive injury or lower limb amputation of artificial knee joint cartilage specimens,HE staining and Saffron O staining,according to the modified Mankin pathological grading score and grouping: 6 cases of normal control group,7 cases of KOA mild degeneration group,14 cases of KOA moderate degeneration group,13 cases of KOA severe degeneration group.The expression of Ghrelin,IL-1? and TNF-? in cartilage of each group was detected by immunohistochemistry.IPP 6.0 image analysis software was used for semi-quantitative analysis and SPSS 21.0 was used software for statistical analysis of experimental results.RESULTS:HE staining and Saffron O staining: There were different degrees of degeneration of chondrocytes and extracellular matrix between the degeneration group and the control group.Immunohistochemistry of cartilage:1.The mean optical density of Ghrelin was 0.0064830.001169 in the control group,0.0046630.000532 in the mild degeneration group,0.0034980.000354 in the moderate degeneration group,and 0.0018340.000590 in the severe degeneration group.There were significant differences between the groups(P<0.05).The expression of Ghrelin was negatively correlated with the Pearson correlation coefficient r=-0.968 in the improved Mankin pathological score.2.The mean optical density of IL-1? was 0.0008170.000352 in the control group,0.0028200.001464 in the mild degeneration group,0.0061170.001306 in the moderate degeneration group,and 0.0085520.001221 in the severe degeneration group.There were significant differences between the groups(P<0.05).The expression of IL-1? was positively correlated with the Pearson correlation coefficient r=0.996 in the improved Mankin pathological score.3.The mean optical density of TNF-? was 0.0008170.000352 in the control group,0.0026680.001227 in the mild degeneration group,0.0060220.000959 in the moderate degeneration group,and 0.0087350.001025 in the severe degeneration group.There were significant differences between the groups(P<0.05).The expression of TNF-? was positively correlated with the Pearson correlation coefficient r=0.988 in the improved Mankin pathological score.CONCLUSION: The expression of Ghrelin was negatively correlated with cartilage degeneration,while the expression of IL-1? and TNF-a was positively correlated with cartilage degeneration.PART ? IN SYNOVIAL MEMBRANE OF DIFFERENT MANKIN PATHOLOGICAL GRADES IN KNEE OSTEOARTHRITISOBJECTIVE: Immunohistochemistry was used to detect the expression of Ghrelin,IL-1? and TNF-? in synovium of different Mankin pathological grades in knee osteoarthritis,and to explore the correlation of Ghrelin,IL-1? and TNF-? in the development of KOA,so as to provide theoretical basis and feasible guidance for clinical treatment.METHODS:Thirty-seven synovial specimens of knee osteoarthritis of different severity and 7 normal synovial specimens of knee joint were selected for routine HE staining.According to the pathological score of Mankin,the specimens were graded and grouped: 7 cases in the normal control group,7 cases in the mild KOA group,15 cases in the moderate KOA group,and 15 cases in the severe KOA group.Immunohistochemistry was used to determine the expression of Ghrelin,IL-1? and TNF-? in synovium of each group.IPP6.0 image analysis software was used for semi-quantitative analysis and SPSS 21.0 was used software analysis of correlation of experimental results.RESULTS:(1)HE staining: Different degrees of synovial hyperplasia,edema,fibrosis and inflammatory cell infiltration were observed in KOA synovial group and normal control group.(2)Immunohistochemistry: 1.The mean optical density of synovial Ghrelin was 0.0086940.000711 in the normal control group,0.0062590.000579 in the mild group,0.0043990.000779 in the moderate group,and 0.0016640.000495 in the severe group.The difference between the groups was statistically significant(P< 0.05),and the expression of Ghrelin was negatively correlated with the Pearson correlation coefficient r=-0.982 of synovial Mankin pathological score.2.The mean optical density of synovial IL-1? was 0.0006300.000231 in the normal control group,0.0019140.000552 in the mild group,0.0033140.000526 in the moderate group,and 0.0047920.000645 in the severe group.There were statistically significant differences between the groups(P<0.05).The expression of IL-1? was positively correlated with the Pearson correlation coefficient r=0.970 of synovial Mankin pathological score.3.The mean optical densitys of synovial TNF-? were 0.0016860.000203 in the normal control group,0.0028630.000509 in the mild group,0.0052420.001057 in the moderate group,and 0.0087670.001019 in the severe group.There were statistically significant differences between the groups(P<0.05).The expression of TNF-? was positively correlated with the Pearson correlation coefficient r =0.973 in the synovial Mankin pathology score.CONCLUSION:The expression of Ghrelin was negatively correlated with synovitis,while the expression of IL-1? and TNF-? was positively correlated with synovitis.
