BackgroundOsteoarthritis (OA) is one of the common diseases hurting human large joints, which can cause progressive degeneration of articular cartilage and synovial changes. Denaturation of the cartilage, exposure of the subchondral bones and the osteoarthritis in the joint edge may occur in the operation. Swelling and paining of the joints can be seen in all clinical patients. There are more than 70% elderly people aged 55 living with osteoarthritis, which increase the situations during the growing ages. In comparison, western countries has higher incidence of the disease, which close to 55%. Tumor necrosis factor-a (TNF-a), osteopontin (OPN), and interleukin-1α (IL-1α) having high expression in osteoarthritis cartilage has attracted attention. However, there is the lack of the research in the synovium expression in patients with osteoarthritis. Joint is enclosed in a synovial cavity. Whether the synovial hyperplasia and inflammatory changes can lead to the degeneration of articular cartilage is to be confirmed by further studies. Nowadays joint replacement surgery is commonly used in the treatment of sever osteoarthritis. However, the presence of trauma in the surgery is large, high cost, increasing ages.and other affecting diseases can cause influence on the operations. Hot spot of future research on pathogenesis of OA from molecular cell biology is how to avoid the occurrence of OA, reduce pain, and improve the quality of patients’ life.ObjectiveStudy on the relationship of tumor necrosis factor -α (TNF-α), interleukin- 1α (IL-la), osteopontin (OPN), OA by detecting contents and Synovialis tumor necrosis factor -α(TNF-α), interleukin-1α(IL-1α), osteopontin (OPN) suction light grey value in TNF-α,OPN and IL-1α.Methods53 synovialis specimen cases of OA patients were selected,32 cases were ope rated by primary total knee replacement, and 21 cases were conducted with arthro scopic debridement.15 cases of normal synovialis samples were taken as the cont rol group. The morphology on synoviaisl tissue estimated by Mankin score system and HE staining of specimens. Expression of TNF-a% IL-la and OPN were det ected by immunohistochemistry. MIAS figure analysis system was used to complet e the quantitative analysis and relative product-moment analysis. SPSS 19.0 softwar e was finally used for statistical analysis of experimental data.ResultsHE staining showed that:The experimental groups OA and the control group were found synovialis hyperplasia and inflammatory changes. Immunohistochemistry data showed that:1. TNF-a average gray values:control group 0.0073±0.0080, mild OA group 0.0587± 0.0196, moderate OA group 0.2017±0.0562, severe OA group 0.4175±0.0832. There is the statistically significant (P<0.01). After the determination of TNF-a content and pathology Mankin score r= 0.901, the content of the expressed TNF-a higher, the pathology Mankin core higher.2. IL-1 average gray values:control group 0.0437±0.0252, mild OA group 0.1137± 0.0236, moderate OA group 0.2219±0.0273,severe OA group 0.0252±0.0318. Among the group there was statistically significant (P<0.01).After the determination of OPN content and pathology Mankin score r=0.871, the content of the expressed IL-la content higher, the Mankin pathology higher.3. OPN average gray value:control group 0.0486±0.0362, mild OA group 0.1100± 0.0573, moderate OA group 0.1722±0.0446, severe OA group 0.2090±0.0618. Among the groups there were statistically significant (P<0.05), After the determination of OPN and pathology Mankin score r=0.854, the content of the expressed OPN higher, the pathological Mankin higher.ConclusionTNF-a, IL-la and OPN were expressed in synovialis, but the molecular weight did not grow with the increasing OA symptoms, which suggested that there are relationships between the molecules and the pathogenesis and progression of OA. There is a positive correlation between the expression of TNF-a, IL-la and OPN and the degeneration of OA.
