Objective:We measured serum sCD163 level in SLE patients and analyzing the relationgship with the clinical features,hope to explore the expression and clinical significance of serum sCD163 in SLE patients,and provide a more convincing evidence for sCD163 to be an effective serum marker for the diagnosis and evaluation of SLE,then provide a new thought wheather sCD163 can be a new therapeutic target for SLE.Methods:We choosed 60 newly diagnosed SLE patients,40 SLE patients who had been treated with glucocorticoids or Immune inhibitors,20 newly diagnosed RA patients and 30 healthy people from Shandong Provincial Hospital from October 2017 to December 2018.Serum sCD163 levels were measured by enzyme-linked immunosorbent assay(ELISA).And we analyzed the changes of serum sCD163 levels in SLE patients before and after treatment.Besides,we studied the relationship between them and their clinical features.Results:Serum sCD163 level in SLE newly diagnosed group was significantly higher than that in healthy control group(P<0.001).Serum sCD163 level in RA newly diagnosed group was significantly higher than that in healthy control group,and significantly lower than that in SLE newly diagnosed group(P<0.001).Serum sCD163 levels were increased in SLE newly diagnosed patients with fever and arthritis(P<0.01,P<0.05),thrombocytopenia,increased IgA and IgE,and increased anti-ds-DNA antibody(800IU/ml)(P<0.01).Serum sCD163 level in SLE treated patients was significantly higher than that in the healthy control group,and significantly lower than that in the newly diagnosed SLE group(P<0.001).Patients with arthritis,thrombocytopenia,increased IgA and IgE,and increased anti-ds-dna antibody concentration(800IU/ml)were reduced(P<0.05),and SLEDAI score was decreased(P<0.01).The concentration of sCD163 in newly diagnosed SLE patients was positively correlated with the concentration of anti-ds-DNA antibody(r=0.379,P=0.01),but not correlated with the SLEDAI score(P>0.05).Serum sCD163 level in treated SLE patients was positively correlated with the concentration of anti-ds-DNA antibody(r=0.458,P=0.003),and positively correlated with the SLEDAI score(r=0.430,P=0.006).For the healthy control group,the accuracy of serum sCD163 in the diagnosis of SLE was relatively high(AUC=0.998,95%CIO.994-1.000).The optimal diagnostic cut-off point was 721.77ng/ml,the sensitivity was 0.983,and the specificity was 1.000.For the RA newly diagnosed group,the accuracy of serum sCD163 in the diagnosis of SLE was relatively high(AUC=0.931,95%CIO.873-0.989).The optimal diagnostic cutoff point was 1018.60ng/ml,the sensitivity was 0.817,and the specificity was 0.900.Conclusions:1.Serum sCD163 level is significantly increased in SLE patients,which may be involved in the immunopathological mechanism of SLE as an inflammatory mediator.2.Serum sCD163 level may be important for the diagnosis of SLE and the differential diagnosis between SLE and RA.3.Serum sCD163 level can reflect the severity of SLE patients newly diagnosed or treated.
