ObjectiveAccording to the theory of TCM phlegm wet constitution theory as the instruction,explore whether there is a correlation between the phlegm wet constitution and patients with upper eyelid puff,and analyze the related factors,to guide patients with upper eyelid puff postoperative nursing and recovery,improve its susceptible constitution,reduce the influence of risk factors,to help patients harvest more satisfying effect of cosmetic surgery treatments.Apply an operative method of directional laminar resection of retro orbicularis oculus fat(ROOF)for correction of upper eyelid heaviness,and observe its clinical effect.MethodsBy the means of the epidemiological investigation and study method,through the network platform questionnaire issued in the form of questionnaire.From January 2018 to January 2019,we investigated 144 patients with upper eyelid puff in affiliated hospital of Nanjing University of Chinese Medicine,then carry out a statistical analysis to explore the correlation and related factors between phlegm wet constitution and puff upper eyelid.112 cases with preoperative diagnosis of puff eyes were operated on the operation.From January 2018 to October 2018,the patients in the plastic and cosmetic department of the affiliated hospital of the Nanjing University of Chinese Medicine were all female,aged between 18 to 37,with an average age of 23.1n order to guide the resection of ROOF and orbital septum fat(OF),the upper eyelid was marked by three areas before the operation:orbital blepharoculi,lateral superior orbital region and inferior orbital blepharoculi.The upper eyelid puff was corrected by the method of Roof directional slice.Blepharoplasty was performed by Septoaponeurosis Junctional Thickening technique.The epicanthoplasty was completed by inverted L-type technique.Drainage skin plate was placed in the incision and postoperative compression bandage was performed.Stitches were removed on the seventh day after surgery.Results1.Among 144 patients with upper eyelid puff,102 patients had phlegm wet constitution(accounting for 70.8%),and 42 patients had non-phlegm-wet constitution(accounting for 29.2%).2.There were significant differences between patients with phlegm wet constitution and non phlegm wet constitution in genetic tendency,personality tendency and exercise habits.(P<0.01)3.There was no difference between patients with phlegm wet constitution and non phlegm wet constitution in upper eyelid swelling degree classification and BMI between groups.(P>0.05)4.The patients was followed 6 months to 12 months after surgery.The heaviness of upper eyelid had improved and all the patients in the group showed satisfaction with the effect.Complication statistics:Three postoperative hematoma,two mild keratitis,two cases sulcus in the left eye,four cases of mild occasional pain in the middle and lateral upper eyelid,four knotting abscess,five local slip,one relapse in unilateral upper eyelid,three narrow double eyelid fold,two asymmetry.Conclusion1.Phlegm wet constitution was associated with upper eyelid swelling.Phlegm wet was one of susceptible factor of upper eyelid swelling.The formation of phlegm wet constitution is related to heredity,personality,exercise,diet and other factors.2.The technique was effective in reducing the heaviness of upper eyelid.Patients showed great satisfaction with the operation result.The technique has clinical popularization and application value.In the operation,anatomical positioning of ROOF resection scope,complete retention of orbital septum and the release and anchor of OF were the key to prevent complications.
