To Observe The Clinical Effect Of Levosimendan In The Treatment Of Acute Heart Failure By Using PiCCO Technique

Objective:A new inotropic drug,levosimendan,can enhance myocardial contractility and alter cardiac function and hemodynamics in patients with heart failure,because the clinical use of this drug is less,and its clinical efficacy and side effects need further observation.In this study,PICCO was used to observe the clinical efficacy and safety of levosimendan,a new positive inotropic drug.Methods: 60 patients with acute heart failure were selected and randomly divided into control group(30 cases)and left Simendan group(30 cases).The observation group in the conventional treatment of heart failure on the application of levosimendan,first with a load of 10ug/kg,10 minutes after the intravenous injection,and then maintain the amount of 0.1ug(kg/min)intravenously,as patients tolerated,no adverse reaction,2 hours after increasing drug dose to maintain 0.2ug(kg/min),remain static drop the total course over 24 hours,the control group,2ug(kg/min)with dobutamine infusion,after 2 hours to 4ug(kg/min)continuous infusion for 24 hours,the total course of treatment.After the treatment,clinical observation was needed for 3days,and the parameters of LVEF,BNP and PICCO were observed.Results:Two groups of patients using Pi CCO technology to monitor the parameters of cardiac function were compared between the third days were compared between the two groups before and after treatment,levosimendan group after treatment,patients with cardiac index left ventricular contractility index,cardiac function index,heart ejection fraction increased significantly reached statistical significance(P<0.05).After the treatment of BNP and cardiac ultrasound assessment of LVEF blood biochemical indexes of levosimendan group were significantly improved(P <0.05).Conclusions:Compared with dobutamine,levosimendan can significantly improve short-term cardiac function in AHF patients,improve left ventricular ejection fraction and improve cardiac systolic function in a short time.The clinical side effects of levosimendan were less,and the incidence of malignant arrhythmia was not increased in short term.
