Objective:The diagnostic value of labial gland tissues at different locations in patients with Sjogren's syndrome was further studied by detecting the focal index of labial gland tissues at different locations,germinal center and expression levels of CD20 and CD3.Methods:1.Collected from Northern Jiangsu People's Hospital of 34 patients with definite diagnosis of SS in all kinds of basic data,including age,course of general condition,dry mouth,dry eye,joint pain,Renault phenomenon such as clinical symptoms,blood cell analysis,rheumatoid factor,complement C1 q,immunoglobulin,blood sedimentation,C3,complement C4,antibody of SSA,SSB antibody,pulmonary function,chest CT,cardiac ultrasound and clinical data.2.The left and right labial gland tissues of the patient were collected,and pathological specimen preparation,HE staining and film reading were completed.The positive criteria of labial gland biopsy were FS?1,to evaluate the infiltration of bilateral labial gland lymphocytes in patients with sclerosing syndrome.3.According to the positive test results of germinal centers in labial gland tissues,patients with SSS were divided into germinal center positive group and germinal center negative group,and the differences in clinical manifestations,laboratory data and involvement of various organ systems between the two groups were compared.4.The expression of CD20 and CD3 in the labial glandular tissues of patients with Sjogren's syndrome was detected by immunohistochemistry,and the expression degree was determined according to the staining intensity and distribution of the sections.The expressions of CD20 and CD3 in the labial glandular tissues at different positions on the left and right sides were compared,and the expressions of CD20 and CD3 werestatistically analyzed in relation to the clinical manifestations,laboratory indicators and the involvement of various organ systems in patients with Sjogren's syndrome.5.Data were imported into SPSS24.0 software for analysis.Results:1.A total of 34 SS patients were collected,with a male to female ratio of 1:33 and an average age of 54±13 years.The median course was 2 years.2.Statistical analysis was conducted on the infiltration degree of bilateral labial gland tissues,and there was no statistically significant difference in the positive rate of labial gland biopsy between the left and right sides(P > 0.05).However,the positive rate of left labial gland biopsy(85.3%)was higher than that of right(82.4%).3.In 10 patients(29.4%),the structure of germinal center was observed in HE staining sections,and in 2 cases,only the germinal center was found on the left side,with no statistically significant difference in the positive rate of germinal center between the left and right sides(P> 0.05).4.Patients with positive labial glandular germinal center were more likely to suffer from Raynaud's disease,musculoskeletal and renal involvement,and the incidence of total system involvement was higher(P< 0.05).The levels of erythrocyte sedimentation rate,complement C1 q and immunoglobulin G in the germinal center positive group were higher than those in the germinal center negative group(P< 0.05).5.CD20 expression was found in labial glandular tissues of 24 patients(70.6%),and CD3 expression was found in labial glandular tissues of 19 patients(55.9%).There was no statistically significant difference in CD20 and CD3 expression between left and right sides(P> 0.05).However,the expression of CD20 and CD3 in the labial glandular tissue of the left side was higher than that of the right side.6.There was no statistical difference between the expression of CD20 and CD3 and the age and course of disease of the patients(P>0.05),but the average age of the patients with CD3 positive was relatively younger,the course of disease was shorter,and the average age of the patients with CD20 positive was relatively older.In laboratory tests,the levels of erythrocyte sedimentation rate,complement C1 q and immunoglobulin Gincreased with the increase of CD20 expression(P< 0.05).Patients with CD20 positive were more likely to have Raynaud's disease,lung and skeletal muscle involvement,and the overall incidence of system involvement was higher than that in the CD20 negative group(P< 0.05).Conclusion:1.In this study,the positive rate of left labial gland biopsy was 85.3%,and that of right labial gland biopsy was 82.4%.The results of bilateral labial gland biopsy showed no significant difference,which can be used for the diagnosis of sjogren's syndrome.2.Patients with positive germinal center in labial gland tissues are more likely to have systemic involvement.3.The expression level of CD20 is correlated with the extraneous manifestations of sjogren's syndrome.The stronger the expression,the more extraneous involvement.4.There were no statistically significant differences in labial gland foci index rating,germinal center positive rate,and CD20 and CD3 expression between the two sides of labial gland tissues,but the positive rate of left labial gland tissues in various examinations was higher than that on the right side.In the absence of special circumstances,left labial gland biopsy could be considered to improve the positive rate of labial gland biopsy.
