| Objective:Stunting is a conmon malnutrition in children.It is the main manifestation of chronic malnutrition in children and endangers children’s early or future health.This paper analyzes the dynamic changes of stunting prevalence among children under seven years old in Dalian from 2010 to 2015,and explores risk factors for stunting by case-control study,in order to provide scientific basis for the development of relevant preventive measures and strategies.Methods:1.According to the medical examination data of children under seven years old in 15 districts and counties in Dalian from 2010 to 2015,the stunting rate of children under seven years old was calculated.2.Case-control study:From January 1st.2015 to May 31st,2015,173 stunting children under seven years old were collected as a case group from the physical examination data of children in various community prevention and control stations in Dalian.According to the ratio of 1:1,children of the same gender,the same administrative district,and normal age were selected to form a control group.Questionnaires were used to collect the general situation of the child,the parental situation,the family’s economic status.the condition of the newborn at birth,and the feeding status of the child.3.Data was entered using Microsoft Excel 2007,and statistical analysis was performed using SPSS 17.0.Line graphs were drawn to show the trend of stunting rate for urban and rural children separately in each year.Univariate and multivariate conditional logistic regression analysis was performed with "stunting" as dependent variable,and stunting independent variables included the general condition of the child,the parental situation,the condition of the newborn,the family econolic status,and the feeding condition of the child.Results:1.Dynamic changes in stunting rate among children under seven years oldThe stunting rate among children under seven years old in Dalian city between 2010 and 2015 fluctuated between 0.21%and 0.46%.In 2011,the stunting rate among boys and girls was both higher,the stunting rate among boys was 0.49%,and the stunting rate among girls was 0.43%.In the past six years,the stunting rate among children under seven years old in rural areas fluctuated between 1.04%-1.29%.In 2010,the stunting rate among rural boys and girls was both higher,1.28%and 1.30%respectively.Between 2010 and 2015,the stunting rate among children under seven years old in rural areas was higher than that in urban areas,which were 5.6 times,2.3 times,5.3 times,3.4 times,2.8 times,and 2.7 times,respectively.In the past six years,there was no significant difference in stunting rate between urban boys and girls.The stunting rate of rural boys was slightly lower than that of girls except 2013.2.That the father’s childbearing age was≥35 years old,the mother’s height was short,the birth length was short,and the infant’s appetite was "middle" or "poor" were the main risk factors for stunting.The stunting rate in children whose fathers’childbearing ages were equal or greater than 35 years was 2.103 times that in children whose fathers’ childbearing ages were smaller than 35 years(95%OR Cl:L283?6.235).Children whose mothers were short stature are prone to be stunting with an OR of 14.580;children with bom short stature are prone to be stunting with an OR of 7.710.Children with moderate appetite are more prone to be stunting than children with good appetite with an OR of 2.300(95%OR Cl:1.079~4.902).The rate of stunting in children with poor appetite is about 3.572 times that of children with good appetite.Conclusion:1.During the period of 2010-2015,the stunting rate anong children under seven years old in Dalian was fluctuating from 0.21%to 1.29%,which was lower than the national level,and the rate in the rural areas were still higher than that in the urban.2.That father’s childbearing age is more than 35 years old,the mother’s height is short,the birth length is short,and the poor appetite of infants and young children wore the main risk factor for stunting among children. |
