| Objective To observe the therapeutic effect of lower extremity Xiaozhong Huoxue Decoction on postoperative swelling of bilateral ankle fracture and the influence to patient’s coagulation indicator.To compare with the conventional control group in reducing and relieving effect on the swelling.To provide an effective solution for early postoperative swelling and to prevent deep veins thrombosis of the bilateral ankle fracture.Methods From January 2016 to October 2017,62 patients with bilateral ankle fracture undergoing surgery in orthopedics department of our hospital were enrolled in this study.They were randomly divided into traditional Chinese medicine group and control group.Surgery option of all the patients who were enrolled are basically the same.The control group was treated by cold packs and ice packs and to elevate the affected limbs in the end of the operation.Oral administration of analgesic tablets for 5 days.Subcutaneous injection low molecule heparin for 10 days.After the first postoperative day they were given intravenous infusion of 20%mannitol 250 ml three days,1 time/d.Guide patients to do functional rehabilitation exercises according to the patient’s postoperative condition.It was mainly included raising the limb to do ankle flexion and extension exercises after operation in the first week.To do ankle and knee flexion and extension exercises in the second week.Chinese medicine group were given lower extremity Xiaozhong Huoxue Decoction treatment in the basic treatment of the control group.Prescription medicine provided by our hospital pharmacy,and unified decoction.Patient was given 1 dose every day and 2 times per dose.Results1.Comparison of ankle swelling after treatment.The swelling circumference of ankle joint was measured at 1 day,3 days and 7 days after operation respectively(see Table 1).Immediately after operation,the difference between two groups was not statistically significant(P=0.365,P>0.05).After 1 day of treatment,there was a significant difference in the swelling degree between the two groups,from4.51±0.74 cm immediately after operation to 3.90±0.54 cm in the Chinese medicine group.The degree of swelling relief was significant(P<0.05).On the 3rd day after operation,the swelling circumference of ankle joint in both groups continued to decrease,which was more obvious than that in the traditional Chinese medicine group on the 1st day after operation.Compared with the control group,the swelling effect of the traditional Chinese medicine group was significantly higher than that of the control group(P<0.05).At 7 days after operation,the swelling regression of the traditional Chinese medicine group was still significantly better than that of the control group.2.Comparison of pain degree in patients.There was no difference in VAS score between patients before operation.After treatment,the VAS scores of lower limbs were statistically analyzed 7 days after operation(see Table 2).The VAS score of TCM group was 2.06±1.06.Compared with the control group(3.65 ±1.47),the score decreased significantly(P<0.05),and the difference was statistically significant(P<0.05).It suggested that the pain degree of the patients in the TCM group was obviously alleviated,and there was a significant difference between the TCM group and the control group.It shows that Chinese medicine treatment can effectively relieve the pain in the trauma area of the patients.3.Comparison of patients’ coagulation indicatorCompared with the change of coagulation indicator of the two groups(see Table 3).The difference coagulation indicator of two groups was not statistically significant after 1 day of the operation(P>0.05).But the difference coagulation indicator of two groups was statistically significant after 7 day of the operation(P<0.05).The coagulation indicator of TCM group was significantly better than that of the control group.That showed Chinese medicine treatment can effectively promote the patient’s coagulation function.4.Comparison of patients’ hospital staysCompared with hospital stays of the control group and TCM group(see Table4).The hospital stays of TCM group was 12.35 ± 1.80 days.The hospital stays of control group was 14.42 ±2.17 days.It was longer than that of TCM group.The difference was signiifcant(P<0.05).That showed Chinese medicine treatment can effectively shorten hospital stays.5.Comparison of therapeutic effectsAfter treatment,in the TCM group there are 4 cases were cured,22 cases were remarkably cured,4 cases were effective cured,and the total effective rate was 96.77%.In the control group there is 1 case was cured,10 cases were remarkably cured,13 cases were effective cured,and the total effective rate was 77.42%(see Table 5).The effective rate of TCM group was higher than that of the control group,and the difference was statistically significant.It shows that Chinese medicine has excellent curative effect on postoperative swelling of bilateral ankle fracture.Conclusion1.Lower limb Xiaozhong Huoxue decoction is composed of Taohong Siwu decoction and Fangji Huangqi decoction.It has efficacy for the benefit of vital qi,activating blood and remove stasis and water swelling.It has good clinical effect in treating postoperative swelling of ankle fracture.It can effectively improve the pain and swelling of the patients.2.Application of lower limb Xiaozhong Huoxue decoction can effectively improve the part of the blood coagulation function in patients with bilateral ankle fracture.3.Use lower limb Xiaozhong Huoxue decoction can promote subsiding swelling of the affected limb and shorten hospital stays.It can not only relieve patients’ pain,but also reduce the economic pressure of patients,so it can be widely applied in clinical practice. |
