Application Of PPH Combined With Modified Ganmai Dazao Decoction In The Treatment Of Rectum Protrusion Constipation(liver-stagnation And Blood-deficiency)

Objective: To observe the clinical effect and feasibility of PPH(Procedure for Prolapse and Hemorrhoids)combined with modified Ganmai Dazao decoction in the treatment of rectum protrusion constipation(liver-stagnation and blood-deficiency).Method: According to the need of the project,all patients with rectal protrusion constipation were included in the study group.56 patients were randomly divided into two groups:28 patients in the treatment group(PPH combined with modified Ganmai Dazao decoction),and 28 patients in the control group(PPH).To observe and record the quantitative score of constipation(western medicine)(the nature of excrement,the difficulty of defecation,the time of defecation,the intervai time and so on)before and after treatment and the quantitative score of rectum protrusion constipation(TCM)in patients with liver stagnation and blood deficiency(main symptoms,minor symptoms)before and after treatment,and to follow up the curative effect of the patients 6 months after operation.The treatment effcet was assessed basised on the data obtained after the medical statistics.Result: 56 people in age,the disease duration,associated with the type of disease had no significant difference.1.The comparison of curative effects between the two groups(1)One month after operation,according to the quantitative score of constipation(western medicine),the total effective rate of the treatment group was 92.86%,and the total effective rate of the control group was 89.29%.The curative effect of the treatment group was better than that of the control group(P<0.05).According to the quantitative scoring standard of rectum anterior protrusion constipation(TCM)of liver-stagnation and blood-deficiency type,the total effective rate of the treatment group was 100.00%,and the total effective rate of the control group was 71.43%.The curative effect of the treatment group was better than that of the control group(P<0.05).(2)6 months after operation,the curative effect of the treatment group was better than that of the control group(P<0.05).2.The comparison between two groups1 month after operation,according to the quantitative score of constipation(western medicine),the therapeutic effect of the treatment group on fecal quality,defecation difficulty degree,defecation feeling,anus and perineum falling feeling and defecation interval was better in the treatment group than in the control group(P<0.05).According to the quantitative score standard of rectum anterior protrusion constipation(TCM)of liver depression and blood deficiency type,the improvement of main(defecation difficulty,post-constipation uncomfortable,abdominal/hypochondria distension)and secondary symptoms(irritable irritability/emotional depression,belching hiccup/less food hiccup,insomnia and many dreams,lip/facial color)in the treatment group was better than that in the control group(P<0.05).3.Intra-group comparisonAccording to the standard of quantitative evaluation of constipation(Western medicine),this group in the all quantified score(Including:fecal quality,defecation difficulty degree,defecation time,defecation feeling,anal clogging feeling,anus and perineum falling feeling,defecation interval time)before and after treatment were obviously improved(P<0.05).In the control group,there was no significant improvement in fecal properties(P > 0.05),but significant improvement in other aspects(P < 0.05).According to the quantitative evaluation criterion of the patients with liver depression and blood deficiency type rectum anterior process constipation(TCM),the treatment group had significant improvement in the main and secondary symptoms(P < 0.05).In the control group,“belching hiccup/less food hiccup”,“insomnia and many dreams”,“lip/facial color” had no significant improvement(P>0.05),and there was significant improvement in the main symptom and secondary syndrome “irritable irritability/emotional depression”(P<0.05).Conclusion: PPH combined with modified Ganmai Da Zao decoction is more effective than single PPH in the treatment of rectum protrusion constipation(liver-stagnation and blood-deficiency)both in the short-term and long-term result.It can improve many uncomfortable symptoms caused by constipation.The observation results showed that PPH combined with modified Ganmai Da Zao decoction in the treatment of rectocele constipation(liver-stagnation and blood-deficiency)the feasibility,practicality and safety,it is worthy of promotion.
