Comparative Analysis Of Delivery Fear And Sleep Status Between Primipara And Second Trimester Pregnant Women

Objective: This study compared the level of fear of childbirth and the status of sleep quality between the first pregnant women and the second pregnant women,explored the relationship between fear of childbirth and sleep quality of the subjects,analyzed the influencing factors of sleep quality of pregnant women,and provided reference for reducing fear of childbirth and improving sleep quality of pregnant women.Methods: In this study,convenience sampling was adopted to select 264 pregnant women who met the standard for questionnaire survey,including 169 first-time pregnant women and 95 second-child pregnant women.The survey tools included:General Situation Inventory,Fear of Childbirth Scale(CAQ),Pittsburgh Sleep Quality Index Scale(PSQI).SPSS26.0 software was used for data analysis.Results:(1)General information: the majority of subjects have bachelor’s degree or above,urban area,family income of 4000 yuan or above,medical insurance,comprehensive prenatal examination and high degree of spouse support.Most of the first-time pregnant women were between 25 and 35 years old(87.6%),most of the second pregnant women were over 30 years old(81%),and most of the last delivery mode was natural delivery.(2)In this study,the score of fear of childbirth in the first pregnant women was(34.86±9.19)points,and the score of fear of childbirth in the second pregnant women was(37.60±3.49)points.There were significant differences in the total score of fear of childbirth and the dimension of fetal health between the first pregnant women and the second pregnant women(P < 0.05).(3)The single factor analysis of fear of childbirth showed that there were differences in education level,working situation,family per capita monthly income,medical insurance,prenatal examination and spouse support.There were significant differences in age,prenatal examination and last delivery mode among the second pregnant women(P < 0.05).(4)In this study,the score of sleep quality of first-time pregnant women was(6.04±2.96),and that of second-born pregnant women was(7.77±3.05).Comparison between first-time pregnant women and second-born pregnant women showed that there were differences in the total score of sleep quality,daytime dysfunction,subjective sleep quality and sleep time,with statistical significance(P < 0.05).(5)Univariate analysis of sleep status showed that there were differences in family per capita monthly income,medical insurance,prenatal examination and spouse support.There were significant differences in gestational age,spouse support and last delivery mode among the second pregnant women(P < 0.05).(6)There was a positive correlation between fear of childbirth and sleep quality in first-time pregnant women(r=0.748),and there was a positive correlation between fear of childbirth and sleep quality in second pregnant women(r=0.526).(7)The results of multiple stepwise regression showed that the fear of childbirth and spouse support were the influencing factors of sleep quality,and the total regression model could explain 57.1% of the variation of sleep quality.The fear of childbirth,gestational age and spouse support were the influencing factors of sleep quality,and the total regression model could explain 50.6% of the variation of sleep quality.Conclusion:(1)Both the first-time pregnant women and the second pregnant women were at a moderate level of fear of childbirth,and the fear of childbirth in the second pregnant women was higher than that in the first pregnant women.(2)The sleep quality of the first pregnant women and the second pregnant women was in the decline level,and the second pregnant women had worse sleep quality than the first pregnant women.(3)The higher the level of fear of childbirth,the worse the sleep quality;The correlation was higher in first-time pregnant women than in second pregnant women.(4)Fear of childbirth and spouse support were the influencing factors of sleep quality in the first pregnant women,while fear of childbirth,gestational age and spouse support were the influencing factors of sleep quality in the second pregnant women.
