ObjectiveTo analyze the clinical efficacy of various capsular tension ring for the implantation of subluxated cataract surgery between 90' and 180° quantificationally.MethodsThe base datum of 17 eyes of 17 patients were collected which were admitted by the Yantai Haigang Hospital and Yantai Aier Hospital from December 2014 to August 2016.The patients were all operated by phacoemulsification and implantation of IOL and capsular tension ring for the reason of 90°-180° subluxation of lens.The pre-install and improved capsular tension rings were implanted in the patients' eyes randomly and the eyes were appointed to A or B group respectively.The best corrected visual acuity(BCVA),intraocular pressure(IOP),visual psychological behavior scores were recorded before surgery.During the postoperative follow-up visit,the best corrected visual acuity(BCVA),intraocular pressure(IOP),anterior chamber deeps(ACD),lean and eccentricity of IOL and other complications were recorded in the postoperative days of 1 day,1 week,and 3 months respectively.The clinical efficacy of different groups could be analyzed.ResultsAmong the follow-up days,all the BCVA of patients were better than pre-operation,showing a significant difference(P<0.05).One day after operation,the BCVA above 0.5 accounted for 4 eyes(50%)in group A and for 5 eyes(55.6%)in group B,presenting in no significant difference between the two groups(P>0.05).One week after operation,the BCVA above 0.5 accounted for 5 eyes(62.5%)in group A and for 7 eyes(77.8%)in group B,presenting in no significant difference between the two groups(P>0.05).Three months after operation,the BCVA above 0.5 accounted for 5 eyes(62.5%)in group A and for 8 eyes(88.9%)in group B,presenting in no significant difference between the two groups(P>0.05).The IOP of the two groups of patients presented in a trend that increasing first and then decreasing to normal.One day after operation,the values of IOP were higher than preoperative,showing a significant difference(P<0.05).The IOP above 21mmHg accounted for 2 eyes(25%)in group A and for 4 eyes(44.4%)in group B,presenting in no significant difference between the two groups(P>0.05).One week after operation,the IOP above 21mmHg accounted for 1 eyes(11.1%)only in group B,presenting in no significant difference between the two groups(P>0.05).The IOP was normal in all patients who were checked 3 months after surgery.The postoperative anterior chamber depth(ACD)in the different times presented significant difference(P<0.05)in both A and B groups with F values were 5.425 and 5.199 respectively and P values were below 0.05.The ACD of both groups increased after 1 week and decreased after 3 months,but higher than 1 day after postoperative,with no statistically significant(P>0.05)between both groups,and from all follow-up time,the ACD of group A was higher than group B,which was also not statistically significant(P>0.05).The difference among title degrees of IOL was statistically significant(P<0.05)in all follow-up time.Both A and B also obtained the same result which was statistically significant(P<0.05).F values were 7.902 and 43.588 respectively.Integrally,both groups of title degrees has increasing trend during follow-up time and A group was higher than B group(F=4.768,P<0.05)as the same as the difference in inter-groups.The difference among eccentricities of IOL was not statistically significant(P>0.05)in all follow-up times.Both A and B also obtained the same result which was not statistically significant difference(P>0.05).F values were 0.125 and 0.485 respectively.Eccentricity was basically stable.The difference of eccentricity between group A and group B was also not statistically significant(P>0.05).And from all follow-up time,there also was no statistically significant difference in eccentricities between the two groups(P>0.05).Other complications:corneal edema was found in one eye postoperatively in A group patients,posterior chamber hemorrhage was found in one eye intraoperatively and corneal edema was found in three eyes postoperatively in B group patients.Cystoid macular edema was found in one patient eye in both groups 3 months after surgery.There were no statistically significant difference(P>0.05)in complications between the two groups.ConclusionThe implantation of capsular tension ring in subluxated cataract surgery between 90'and 180° can effectively maintain the postoperative clinical efficacy.The implantation and fixation of improved tension ring is better in stability maintenance of the IOL,but may increase hemorrhage and corneal edema in intraoperative and postoperative.
