Purpose: In order to investigate the neonates with mother's colostrum in the oral cavity buccal mucosa as an immunotherapy for premature infants during the hospitalization,neonatal necrotizing enterocolitis,neonatal late-onset sepsis,and hospitalization time and the impact on the weight gain of children.Then evaluate the clinical effect of colostrum oral instillation.Method: This study belongs to an quasi-experimental study.With the convenience sampling method,we select infants with weight less than 2500 g and gestational age less than34 weeks in the NICU from February 2017 to March 2018 as our subjects of the study.According to the consent of the family members and whether they can provide fresh colostrum for the mothers of the children,they were divided into two groups.One group that can provide was the observation group and another group could not be the contrast group.There was no significant difference of statistics on the age,delivery method,number of births,and perinatal complications between the two groups of preterm infants(P>0.05).There was no significant difference of statistic in the preterm infants between the two groups in terms of gender,1-minute Apgar rating score,5-minute Apgar rating score,gestational age,birth weight,as well as mechanical ventilation(P>0.05).The observation group used a uniform 1 ml sterile syringe.Before each operation,0.2 ml of mother's fresh colostrum was taken from the patient,and each side of the buccal mucosa was infused with colostrum 0.1 ml so that the colostrum was evenly distributed in the buccal mucosa,the upper palate,and the gums,tongue and other oral mucosa.The contrast group had the same treatment and nursing methods except that the colostrum was not instilled into the colostrum.The feeding process,the incidence of neonatal necrotizing enterocolitis,the incidence of late-onset neonatal sepsis,the total length of hospital stay,the time to restore birth weight,and the weight gain of the infants have compared between the two groups.Statistical analysis was performed by using spss22.0 software for data analysis.The difference was statistically significant at P<0.05.Result:(1)There was a significant difference between the observation group and the contrast group(P<0.05)in the beginning of oral feeding in terms of gestational age and time,complete oral feeding of gestational age and time,and total gastrointestinal gestational age andtime(P<0.05).(2)The incidence of neonatal necrotizing enterocolitis was not statistically significant between the observation group and the contrast group(P>0.05).(3)The incidence of late-onset sepsis in the observation group compared with the contrast group was statistically significant(P<0.05).(4)The total length of hospitalization in the observation group compared with the contrast group was statistically significant(P<0.05).(5)Obviously,time for recover birth weight in the observation group was significantly higher than that in the control group(P<0.05),but the difference in weight between the start of oral nutrition,oral feeding,complete oral feeding,and total enteral nutrition.There was no statistically significant difference(P>0.05).There was no significant difference leaving hospital weight and weight of increase rate(P>0.05).Conclusion: Oropharyngeal colostrum administration is beneficial for premature infants to advance the feeding process,it can reduce the incidence of neonatal late-onset sepsis,shorten hospitalization time,and shorten the time to restore birth weight.Thus it can be widely used in clinical practice.
