Background:For elderly bedridden patients,the morbidity and mortality of lung infection are high because of immobility and decreased immune function,which seriously affects their physical and mental health.Standardized preventive nursing interventions can effectively reduce the incidence of lung infection,but due to lacking of care interventions for lung infection among elderly bedridden patients,lacking of evidence-based basis for routine nursing care,poor compliance with caregivers,and other reasons,the incidence of lung infection in elderly bedridden patients is still at a high level.Therefore,this study is aimed at carrying out standardized preventive nursing interventions for elderly bedridden patients and exploring the effect of intervention on the incidence of lung infection in elderly bedridden patients.Objective:To compare the incidence of lung infection in the elderly bedridden patients between the intervention group and the control group after the standardized preventive care intervention.Methods:Quasi-experimental study with historical control was used in this study,with patients selected conveniently from 25 hospitals in 6 provinces and cities across the country.The 7324 cases from November 2015 to June 2016 served as the control group,while 7323 cases from November 2016 to July 2017 as the intervention group.The routine care were given to the control group,in contrast,the standardized preventive care intervention measures were given to the intervention group.The self-designed case report form was used to collect the occurrence of lung infection and the implementation of related measures during the hospitalization among elderly bedridden patients.The self-designed nurses' knowledge and attitudes questionnaire on the prevention and nursing of lung infection among bedridden patients was used to measures nurses' knowledge and attitudes,while quality control table collected the implementation rate of measures.Results:We completed data collection of 14647 elderly bedridden patients in this study,including 7323 in the intervention group and 7324 in the control group.(1)After standardized preventive care intervention,the overall incidence of lung infection in bedridden patients decreased from 7.73%to 5.79%,and the difference was statistically significant(p<0.05).(2)Subgroup analysis suggested that standardized preventive nursing interventions had better preventive effects on lung infections among bedridden patients in different length of bedridden and different age groups.(3)After standardized nursing training,the average score of nurses' knowledge on lung infection prevention in the intervention group was increased from 9.95± 1.77 before intervention to 10.54± 1.70(p<0.001).The average score of attitude improved from 27,20±2.89 before intervention to 27.37±2.83(p<0.05).The implementation rate of oral care measures in mechanical ventilation patients increased from 89.76%(before intervention)to 94.52%(after intervention)(p<0.05);the implementation rate of measures to assist patients with sputum excretion increased from 69.88%to 82.65%before intervention(p<0.001).(4)The overall compliance of standardized preventive care interventions among nurses was 93.98%.Conclusions:Standardized preventive nursing intervention can effectively reduce the incidence of lung infection in bedridden patients in the elderly,and it is worthy of being applied and popularized in clinical elderly bedridden population.
