ObjectiveDistal radial fractures are more common,mostly in older women.Most of the young and middle-aged are traumatic violence,usually occurring in the 2-3 cm distal radius of the humerus,most of which are associated with ankle joint and lower ankle joint injury.In clinical treatment,depending on the fracture condition of the patient,the appropriate conservative treatment or surgical treatment is selected.However,there is some controversy about the choice of intraoperative fixation,especially the complex distal radius fracture involving the articular surface.The clinical effects of two kinds of plate treatments involving distal fractures of the articular surface were compared to select a more effective internal fixation plate.MethodsIn this study,a retrospective controlled study was conducted.From June 2016 to May 2017,the time series of patients were selected.42 patients with distal radius fractures were selected as subjects,and they were divided into common t-type locking plates according to different surgical methods.Ordinary t-lock plate internal fixation,n = 18 cases)and multi-axial plate(multi-axial locking plate internal fixation,n = 24 cases),comparing the clinical healing time,complication rate,and complication rate of the two groups of patients The Gartland-Werley wrist function scores were scored 6 months after surgery and compared using statistical software.Results1.The general data of the two groups were statistically different,and the difference was not statistically significant(P>0.05).2.The clinical healing time of the fractures of the multi-axial locking plate group was(49.3±7.6)d,which was shorter than that of the conventional T-lock plate group(57.6±8.5)d,and the difference was statistically significant(P<0.05).The complication rate of the multi-axial locking plate group was 4.2%,which was lower than that of the ordinary T-lock plate group(11.1%),and the difference was statistically significant(P<0.05).At 6 months after operation,according to Gartland-Werley Wrist Function Score,the excellent and good rate of wrist joint function of multi-axial locking plate group was87.5%,which was higher than that of ordinary T-type locking plate group(77.7%),the difference was statistically significant(P<0.05).ConclusionFor the distal radius fracture involving the articular surface,especially in elderly patients with severe comminuted and osteoporosis,multi-axial locking plate is recommended for open reduction and internal fixation.For young patients with better bone quality and less severe comminution,ordinary T-locking steel plates can be considered.No clinical treatment has an absolute advantage.According to the type and severity of the fracture,combined with the patient's physical condition and economic situation,comprehensive analysis should be made to develop a suitable treatment plan.
