| Objective: The author summarized Professor Lan Qingqiang's academic thought of treating chronic Hepatitis B with self-made Pinggan decoction on the basis of the treatment idea of coordinating the internal environment and its relationship with the external environment in order to find a new balance in“the Internal Canon of Medicine”,and observed the clinical effect of Pinggan decoction combined with antiviral therapy on chronic hepatitis B.Method:All the cases collected in this study came from out-patient department of Professor Lan and were diagnosed and treated on the basis of the treatment concept of coordinating the internal environment and its relationship with the external environment in order to find a new balance.78 patients were included in the trial and divided into two groups according to the principle of random distribution.The treatment group(39 cases)was treated with Pinggan decoction combined with antiviral therapy,while the control group(39 cases)was treated with antiviral therapy alone.The antiviral drugs in both groups were treated with Entecavir Tablets.A course of treatment is 3 months,a total of 2 courses,the total treatment time is 6 months.Liver function index,Hepatitis B two-and-a-half pairs,hepatitis B virus DNA measurement,four items of hepatic fibrosis and safety index were measuredin the first visit and after 2 treatment courses(24 weeks).The changes of the above-mentioned indexes and the integral of TCM syndrome were observed to compare the clinical effects of the two treatment regimens.Results:(1)Clinical comprehensive curative effect: The total effective rate was 89.4% in the treatment group and 68.4% in the control group.The total effective rate in the treatment group was higher than that in the control group,with statistical difference(P<0.05).(2)TCM syndrome integral: the difference between the two groups was statistically significant before and after the treatment(P<0.05);after treatment,the integral of TCM syndrome in the treatment group was lower than that in the control group,and the difference was significant(P<0.05).(3)Biochemical indicators: After treatment,ALT,AST and TBIL decreased significantly,the decrease of treatment group was greater than that of the control group,and the difference was statistically significant(P<0.05).Compared the indexes of hepatic fibrosis before and after treatment between the two groups,the levels of HA,LN,IV-C and PC-III in the two groups were lower than those in the control group,and the difference was statistically significant(P<0.05);the index level of hepatic fibrosis in the treatment group was significantly lower than that in the control group after treatment,and the difference was statistically significant(P<0.05),indicating that the treatment group was superior to the control group in improving hepatic fibrosis.(4)Negative conversion ratio of HBeAg: there was no significant difference in the negative conversion rate of HBeAg between the two groups before and after treatment(P>0.05).(5)Negative conversion ratio of HBV-DNA: compared with the control group,the HBV-DNA in the treatment group was significantly higher in the treatment group than in the control group(P<0.05).(6)After treatment,the quality of life of the treatment group was significantly higher than that of thecontrol group,the physiological,psychological and total scores of the treatment group were significantly higher than those of the control group,and the difference was statistically significant(P<0.05).Conclusion: Professor Lan treated chronic hepatitis Bbased on the concept of coordinating the internal environment and its relationship with the external environment in order to find a new balance and under the guidance of Pinggan Decoction in this theory,so as to significantly relieve the clinical symptoms of patients,and contribute to improve liver function and resist hepatic fibrosis,which has a certain auxiliary effect on reducing HBV-DNA load,has some advantages in improving the quality of life of patients,and is worth popularizing and applying in clinic. |
