Objective:Under the guidance of the concept of rapid rehabilitation surgery,the traditional thread-drawing therapy and pad pressure therapy are combined to innovate the treatment of pilonidal sinus.By comparing the overall curative effect,operation time,postoperative indicators and recurrence of thread-drawing combined with pad pressure therapy with Karydakis flap,this paper explores whether this method can accelerate wound healing,reduce postoperative pain,reduce complications and recurrence rate under the premise of complete excision of the lesion,and provides a new surgical method for clinical treatment of this disease.METHODS:Thirty pilonidal sinus patients who met the criteria included in this study were selected and randomly divided into treatment group(using thread-drawing combined with pad pressure therapy)and control group(using Karydakis flap)with 15 in each group.The two groups were observed,recorded and analyzed in terms of cure rate,operation time,postoperative indicators(pain,total hospitalization time,wound healing time,recovery of working and learning time),postoperative complications and recurrence rate.Result:In this study,there was no significant difference in cure rate,operation time,total hospitalization time,wound healing time,recovery of working and learning time between the treatment group and the control group(P>0.05);the treatment group was superior to the control group in terms of postoperative pain(P<0.05);in terms of postoperative complications,4 patients in the control group had incision effusion,and 1 patient had secondary bleeding.In the treatment group,only 2 patients had incision dehiscence.During the follow-up period,there was no recurrence in the treatment group,and 2 patients in the control group had recurrence and underwent reoperation.Conclusion:Thread-drawing therapy combined with pad pressure therapy achieved satisfactory clinical efficacy,and was superior to Karydakis skin flap transfer in relieving postoperative pain.It also shows potential value in reducing postoperative complications and preventing recurrence.This study inherits and carries forward the traditional Chinese medicine treatment,ensures the curative effect,optimizes the treatment path of pilonidal sinus,provides a new idea for the surgical treatment of pilonidal sinus,and is worthy of further clinical research.
