Background:Complex anal fistula is a refractory anorectal disease. Patients often have to receive several operations, which not only adds to the financial burden on patients and the society but also harms their physical and mental health, thus affecting their quality of life. Tunnel thread-drawing therapy is a minimally invasive surgery for treating complex anal fistula, but its clinical efficacy and influence on patients' quality of life still await further assessment.Objective:To investigate the clinical efficacy of tunnel thread-drawing therapy for treating complex anal fistula and its impact on patients' quality of life.Methods:A total of 60 patients diagnosed with complex anal fistula were recruited and randomly divided into a treatment group and a control group, each of which included 30 patients. Tunnel thread-drawing therapy was given to patients of the treatment group while incision-thread-drawing procedure was performed on those of the control group. According to clinical indexes of the experiment, patients'conditions were recorded during the period from the surgery to three months after wound healing. All results were recorded in a quantitative manner and then processed by using statistical methods. A comparison was made between the two groups in terms of cure rate, recurrence rate, fever, pain, effusion, wound healing time, length of stay, hospitalization expenses, anal function, quality of life, etc.Results:The patients who had received the tunnel thread-drawing therapy showed better clinical outcomes than those who had received the incision-thread-drawing procedure regarding postoperative pain, effusion, wound healing time, length of stay, hospitalization expenses, anal function, and quality of life. There is a statistical difference between the two groups (P<0.05).Conclusions:Tunnel thread-drawing therapy has an evident therapeutic effect in treating complex anal fistula. Although it has a cure rate similar to incision-thread-drawing procedure, tunnel thread-drawing therapy features less postoperative pain, faster wound healing, shorter length of stay, lower hospitalization expenses, smaller influence on anal function and patient's quality of life. Moreover, it meets the requirements of minimal incision, fast recovery, low recurrence rate and good quality of life by modern medicine. With its relatively simple operations, tunnel thread-drawing therapy should be widely put into clinical practice.
