Study On The Relationship Between TCM Syndrome Types And Disease Severity Of Bronchiectasis

Objective: According to FACED score,the severity of bronchiectasis in patients with acute exacerbation was evaluated,and the distribution of disease severity in patients with phlegm-heat obstructing lung syndrome and liver-fire invading lung syndrome was summarized.To explore the relationship between TCM syndrome types and disease severity of bronchiectasis.Methods: By collecting and collating the basic information,TCM syndrome scores,auxiliary examination and other relevant data of the patients with acute exacerbation of bronchiectasis treated in the ward of the fourth Clinical Medical College of Xinjiang Medical University from January 2017 to December 2018,According to FACED score to evaluate the severity of bronchiectasis,to observe the distribution of various indexes in patients with phlegm-heat obstructing lung syndrome and liver-fire invading lung syndrome,and to explore the relationship between the two TCM syndrome types and the severity of the disease.Results: 1.Among the 182 patients with bronchiectasis,there were 69 males and 113 females,153 cases under 70 years old,29 cases?70 years old,56 cases with pulmonary function FEV1% pred < 50%,126 cases?50%,42 cases with Pseudomonas aeruginosa infection.There were 140 cases of Pseudomonas aeruginosa infection.Pulmonary HRCT showed involvement of pulmonary lobe > 2 lobe in 103 cases,?2 lobe in 79 cases,dyspnea score >2 in 78 cases and?2 in 104 cases.2.There were 95 cases(52.20%)with syndrome of phlegm-heat obstructing the lung and 87 cases(47.80%)with syndrome of liver-fire invading lung.3.According to FACED score,the severity of bronchiectasis was evaluated in 109 patients(59.89%),42 patients(23.08%)and 31 patients(17.03%).4.There was no significant difference in Pseudomonas aeruginosa infection between the two TCM syndrome types(P>0.05).There were significant differences in cumulative pulmonary lobe number,pulmonary function(FEV1/FVC,FEV1%pred)and mMRC scores between the two TCM syndrome types(P < 0.05).There were significant differences in pulmonary HRCT between the two TCM syndrome types(P<0.05).5.The comparison of disease severity between the two TCM syndrome types showed that P<0.009(P<0.05),and the difference was statistically significant.Conclusion: 1.Compared with the patients with phlegm-heat obstructing the lung syndrome and the liver-fire invading the lung syndrome,the lung HRCT showed that the cumulative lung lobe number was more,the lung function was worse,and the severity of dyspnea was more serious.2.The mild patients with liver-fire invading lung syndrome were more than those with phlegm-heat obstructing lung syndrome,and the severe patients with phlegm-heat obstructing lung syndrome were more than those with liver-fire invading lung syndrome.3.The severity of disease in patients with phlegm-heat obstructing lung syndrome was more serious than that in patients with liver-fire syndrome.
