Objective:To evaluate the predictive value of C-reactive protein/albumin ratio(CAR)for postoperative complications of pancreaticoduodenectomy(PD).Methods:Retrospectively collected clinical data of 134 patients undergoing pancreaticoduodenectomy(PD)in the First Affiliated Hospital of Xinjiang Medical University from 2015 to 2018,including general conditions,postoperative complications,and postoperative 1,3,5 days of CRP and Alb and calculate the ratio,draw the receiver operating characteristic(ROC)curve to determine the critical values of CAR,CRP,PCT,Alb and calculate the sensitivity,specificity,and area under the curve of each factor for early prediction of postoperative complications.Results:There were no significant differences in general conditions such as gender,age,BMI,ASA score,tumor nature,tumor diameter,operative time,intraoperative blood loss,diabetes history,jaundice history,and drinking history.Of the 134 patients,38 had POPF,32 had abdominal infection,5 had biliary fistula,63 had DGE,13 had PPH,8 had wound infection,and 1 had chyle leakage.The values of CAR in the pancreatic fistula and abdominal infection group were significantly higher than those in the non-complication group at 1d,3d and5 d,P<0.05.The difference was statistically significant.The sensitivity of POF on the 3rd and 5th postoperatively was 79.95% and 78.95%,and the specificity was 86.46% and60.42%,respectively.The sensitivity of predicting abdominal infection was 75.00% and62.50%,and the specificity was 81.37% and 72.55% respectively,the result is better than simply using CRP or PCT.Conclusion:The postoperative C-reactive protein/albumin ratio can better predict the infectious complications after pancreaticoduodenectomy,and the effect is better than CRP or PCT alone.
