| Objective: To evaluate the predictive value of Neutrophil lymphocyte ratio(NLR)for the severe postoperative complications associated with pancreaticoduodenectomy(PD).Methods: The clinical data of 137 patients who underwent pancreaticoduodenectomy at the Department of Pancreas-thyroid Surgery of our hospital from October 2017 to October 2020 were retrospectively analyzed.Univariate and multivariate logistic regression analysis were used to establish risk factors of postoperative complications.Receiver operating characteristic(ROC)curves were plotted to analyze the predicted values of NLR for the severe postoperative complications associated with PD.Results: Of the 137 patients enrolled,22(16%)had serious postoperative complications.Univariate analysis revealed that POD5 drain amylase 、 POD5 NLR were the influencing factor of postoperative complications(P < 0.05).Multivariate analysis showed that the POD5 NLR value was an independent risk factor for severe complications after PD(OR=1.153;95%CI : 1.058-1.255;P=0.001).ROC curve analysis revealed that POD5 NLR is of great value in predicting the occurrence of severe complications after PD(P<0.05),and the area under the curve of POD5 NLR was 0.777.The cut-off value of POD5 NLR was 8.6,the corresponding sensitivity is77.3% and the specificity is 71.3%.Conclusion: POD5 NLR is a predictor of severe complications after PD,which can guide early clinical detection and intervention,so as to improve the short-term prognosis after PD. |
