OBJECTIVE:The randomized controlled clinical study was designed to determinewhether the strategies of drain insertion can influence the incidence ofpostoperative complications.METHODS:From March2013to February2014, a total of56patients whounderwent pancreaticoduodenectomy by a medical team at Sino-JapaneseFriendship Hospital of Jilin University, Xinmin School District, wereenrolled in this study. In order to assess the value of prophylactic drains,and determine whether the strategies of drain insertion can influence theincidence of postoperative complications, we prospectively assigned thepatients into2groups: Extubation all at once (group A, n=28), the tubeswere removed all at once when there was no drainage; withdrawal tubegradually (group B, n=28), since the postoperative day2, the tubes werewithdrew gradually every other day, when there was no drainage thetubes could be removed. Postoperative complications in the2groupswere compared.RESULTS:A total of56patients who underwent pancreaticoduodenectomy wereenrolled in this study, in which group A and B had28patients separately.Although the rate of pancreatic fistula was lower in group B (17.86%) than in group A (39.29%), but the difference was not significant (P=0.076). The incidence of intra-abdominal infections was significantlylower in group B (7.14%) than in group A (35.71%), and it had astatistically significant difference (P=0.009). The incidence of bleedingin group B (0%) was significantly lower than that in group A (14.29%),and it had a statistically significant difference (P=0.038).CONCLUSION:That the tubes were withdrew gradually could reduce the incidence ofintra-abdominal infections and bleeding complications with pancreatichead resection.
