Objective: By exploring the predictive value of C-reactive protein to albumin ratio(CAR)combined with Grace score for short-term major adverse cardiovascular events(MACE)in patients with unstable angina(UA),we aim to provide a reference index for short-term prognosis of patients with UA.Methods: A total of 178 patients diagnosed as UA in cardiovascular medicine of the fourth affiliated hospital of CMU were selected.Based on the optimal cut-off point of CAR determined by the ROC curve(0.0441),patients were divided into the high-value group(62 cases)and the low-value group(116 cases),and the basic data between the groups and the occurrence of MACE were compared.Divided patients into MACE group(10 cases)and non-MACE group(168 cases)based on the occurrence of MACE,and the basic data between the groups were compared.Used univariate and logistic multivariate logistic regression analysis to identify the risk factors for MACE events.ROC curve was used to evaluate the predictive value of CAR combined with Grace score for MACE.Results: 1.The high CAR group had higher levels of white blood cell(P=0.013),neutrophil(P=0.012),higher incidence of recurrent angina pectoris(P=0.036)and re-revascularization(P=0.025).The age(P=0.000),neutrophil to lymphocyte ratio(P=0.004),C-reactive protein(P=0.000),Grace score(P=0.023)and incidence of MACE(P=0.001)of patients were higher significantly,albumin(P=0.002)was significantly lower compared with the low CAR group with statistically significant.2.The MACE group had higher levels of white blood cell(P=0.023),neutrophil(P=0.013),C-reactive protein(P=0.003),CAR(P=0.002)compared with non-MACE group with statistically significant.3.Univariate analysis of the occurrence of MACE indicated age, heart rate, white blood cell, neutrophil,CAR and Grace score were independently correlated with the occurrence of short-term MACE in patients with UA(all P<0.05).4.Logistic multivariate analysis of MACE occurrence indicated CAR(P<0.05)and Grace score(P<0.0001)were statistically significant.5.ROC curve analysis the area under the ROC curve of MACE events in UA patients within 3 months after the onset of UA was 0.787,0.964,0.972 respectively, and the predictive value of the two combined was higher than that of the single prediction, with statistically significant(P<0.05).Conclusion:CAR and Grace score can independently predict the short-term occurrence of MACE in UA patients, and the combination of CAR and Grace score improved the predictive value of MACE in patients with UA than both separate.
