Objectives1.Understand the current status of sleep quality of primary caregivers of patients with liver cancer.2.Understand the current status of the burden of major caregivers of patients with liver cancer.3.To analyze the correlation between sleep quality and burden level of primary caregivers of patients with liver cancer.MethodsThis study is a descriptive study.The researchers used the target sampling method to select 160 primary caregivers of liver cancer patients hospitalized in a third-grade hospital in Changsha from January to August 2018.Informed consent was obtained with the consent of the investigator.The patient's general data,general caregiver's general data,and the Pittsburgh Sleep Quality Index(PSQI)and Caregiver Burden Inventory(CBI)scales were used to assess sleep quality and burden levels using a self-compiled scale.PSQI consists of seven dimensions:sleep quality,sleep time,sleep efficiency,sleep disorder,sleep time,hypnotics and daytime dysfunction.The score of each component is 0?3,the cumulative score of each component is 21 points of PSQI,and the score of 7 points is the limit for evaluating normal sleep quality.The higher the score,the worse the sleep quality.CBI consists of five dimensions:time-dependent load,development-limited load,physical load,social load and emotional load.Each item is divided into 0?4 points according to the weight of the load,and the total score of the scale is 0?96 points.The higher the score,the heavier the care load.Data were entered by double-blind method and statistical analysis was performed using SPSS21.0 software.The count data is described by frequency,percentage,and measurement data using mean and standard deviation.The T-test or analysis of variance was used to compare the normal and variance-measured data sets.The comparison of skewed or variance-measured data was performed by rank sum test.The effects of patients and caregivers on sleep quality and burden level were analyzed by single factor and multiple linear regression analysis.The normal distribution of sleep quality and burden level correlation was analyzed by Spearman rank test.The test level was a=0.05,and the difference was statistically significant when P<0.05.Results1.The primary caregiver of the liver cancer patients,Sleep quality the Pittsburgh Sleep Quality Index(PSQI)score was 8.41±3.3 9(mean1standard deviation),and 95 primary caregivers of liver cancer patients(59.75%)of the caregivers PSQI>7 points.The grading scores of PSQI from high to low are sleep efficiency,daytime dysfunction,sleep time,sleep quality,sleep disorders,sleep time,hypnotic drugs.Univariate analysis of patient factors(whether the disease metastasizes,self-care ability,whether surgery,or pain),caregiver factors(type of care,health status,average daily care time,ability to perform nursing work),economic factors(medical expenses)There was a statistically significant difference in the PSQI score between the stress and the monthly income of the family members(P<0.05).Logistic regression analysis of the main factors affecting the sleep quality of primary caregivers of liver cancer is the average monthly income of the caregiver family.2.The main caregiver's Caregiver Burden Inventory(CBI)scale was 65.3±17.5 points(mean±standard deviation),of which 92 primary caregivers of liver cancer patients(57.5%)were severe burden,62 primary caregivers of liver cancer patients(38.75%)were moderate burden,and 6 primary caregivers of liver cancer patients(3.75%)were mild,burden.The high-to-low latitude scores of CBI are development-limited load,physical load,affective load,time-dependent load,and social load.Univariate analysis of patient factors(whether the disease has metastasized),caregiver factors(gender,work status,good relationship with patients,ability to perform nursing work,personality traits,education level,health status),economic factors(medical cost pressure).There was a statistically significant difference between the CBI score and the CBI score(P<0.05).Logistic regression analysis of the main influencing factors of the caregiver's burden level:whether the patient's disease has metastasis,the caregiver's gender and the relationship between the caregiver and the patient.3.The data was skewed by the normality test and analyzed by SpearmanRelated rank correlation.The care quality of the caregiver was positively correlated with the burden level.That is,the quality of the caregiver's sleep is negatively related to the level of burden.(The higher the sleep quality score,the worse the sleep quality,the more likely the sleep disorder will occur).The Spearman rank coefficient was 0.333(P<0.01).There was also a positive correlation between the 7 dimensions of caregiver sleep quality and the total score of burden level(P<0.05).The Spearman rank correlation coefficients were 0.322,0.163,0.195,0.176,0.297,0.245,0.165,respectively.Conclusions1.Nearly 60%of the primary caregivers of liver cancer have sleep disorders.2.The main caregivers of liver cancer are at the moderate and severe burden level.3.The average monthly income of family members is the main influencing factor of sleep quality of liver cancer caregivers.4.Whether the patient's disease is transferred,the gender of the caregiver,and the good relationship between the caregiver and the patient are the main influencing factors of the burden level of liver cancer caregivers.5.Sleep index(PSQI)of the main caregivers of liver cancer was positively correlated with CBI score(p<0.01).That is,the quality of the caregiver's sleep is negatively related to the level of burden.The higher the burden level of caregivers,the higher the sleep index score,the lower the sleep quality.
