Diagnosis And Treatment Of Special Cardiac Tumor

Objective:Summarize the experience in the diagnosis and treatment of special cardiac tumors.Methods:A retrospective analysis of 3 cases of special cardiac tumor(cardiac myxoma)admitted in Second Hospital of Shanxi Medical University from April 2010 to July 2017:1.intracardiac leiomyomatosis.The patient,42 years old,has the symptoms of heart failure when she is admitted to hospital.It has a history of "uterine myoma".Primary diagnosis was performed by echocardiography and cardiac MRI.Combined obstetrics and Gynecology,vascular surgery,combined thoracoabdominal incision for one stage radical operation.The postoperative recovery was good,and no recurrence was found in the follow-up.2.Primary Cardiac Synovial Sarcoma.A 39 year old male was admitted to hospital with dyspnea and heart murmur.The first diagnosis was performed by echocardiography,and the chest CT showed pulmonary metastasis.Tumor resection was performed under conventional cardiopulmonary bypass,and pulmonary metastasis was removed after extracorporeal circulation was removed.After three courses of chemotherapy,there was no recurrence of tumor in situ,and the tendency of double lung metastasis was increased.3.cardiac papillary fibroelastoma.A 58 year old woman was admitted to the hospital with headache and dizziness,and there was no obvious heart failure.The initial diagnosis by echocardiography,cranial CT clear cerebral infarction complications.Resection of the papillary and chorda tendons of the mitral valve through extracorporeal circulation was performed with mitral valvuloplasty.The postoperative recovery was good,and the heart function grade I was followed up 30 days after the operation.Conclusion:The incidence of special cardiac tumor(cardiac non myxoma)is very low,and the diagnosis and treatment are difficult.The clinical symptoms of the patients are often complex and changeable,and there is no obvious specificity,which is not very helpful for the diagnosis of the disease.Some related previous history is of great significance to the diagnosis.Echocardiography is the primary choice for the diagnosis of a special heart tumor.The CT heart MRI with the chest can better judge the characteristics of the tumor.Surgical treatment is the first choice for most patients.No matter benign or malignant,surgery has certain positive significance.According to the characteristics of tumor,specific surgical plan can be worked out.
