Cardiac hemangioma is a very rare primary tumor of the heart and constitutes only 2.8% of primary cardiac tumors. Meanwhile, Coronary-pulmonary artery fistula is also a rare entity of congenital heart disease. Owing to their rarity, our knowledge about these two kinds of disorders is derived from case reports, lack of systemic recognition. Herein, we present the case of a 45-year-old symptomatic male with a rare cardiac hemangioma. During assessment after admission, coronary-pulmonary artery fistula was diagnosed by computed tomographic angiography of coronary artery. As far as we know, it is the first case in which cardiac hemangioma has been found to co-exist with coronary-pulmonary artery fistula. Surgery remains the most effective form of therapy. In addition, in patients with heart issues, computed tomographic angiography of coronary artery has particularly advantages that can reduce the risk of second operation and missed diagnosis.
