Objective:Henoch-Schonlein purpura(HSP)is the most common vasculitis.It is often manifested as multiorgan dysfunction.The pathogenesis is complicated and not completely understood.The study is to investigate the relevance between Th1/Th2 polarization and different types of HSP,and observe the influence of compound glycyrrhizin injection on Th1?Th2 of HSP and its therapeutic effect.Methods:1.90 patients with Henoch-SchonIein Purpura were selected in the department of dermatology of the affiliated hospital of Chengde Medical College.And IL-2?IFN-? and IL-4 in blood plasma of these patients were determined by ELISA.2.Thirty patients with renal injury and thirty patients with gastrointestinal injury were selected as the research object,which were named as purpura renal injury group and purpura abdominal type group.30 patients with simple purpura were selected as the control group in the same period.Compared the levels of IL-2?IFN-??IL-4 in different groups.3.These 90 cases were randomly divided into compound glycyrrhizin injection treatment group and without compound glycyrrhizin injection group(control group)in 1:1 ratio at the same time.The control group were given the basis conventional treatment(Vitamin C,rutin,antihistamines,etc)for 7days.The treatment group on the basis of conventional treatment was given different doses of compound glycyrrhizin injection according to age and weight,for 7days.Detected and compared the levels of IL-2?IFN-? and IL-4 before and after the treatment.Compared two groups of patients with total effective rate.Results:1.The levels of IL-2?IFN-? in the purpura renal injury group and the purpura abdominal type group were significantly lower than control group(P<0.05).But the differences of IL-2?IFN-? between the purpura renal injury group and the purpura abdominal type group were not statistically significant(P>0.05).2.The level of IL-4 in the purpura renal injury group and the purpura abdominal type group was significantly higher than control group(P<0.05);But the difference between the purpura renal injury group and the purpura abdominal type group was not statistically significant(P>0.05).3.The levels of IL-4 after the treatment in the compound glycyrrhizin injection treatment group and without compound glycyrrhizin injection group were both lower than before(P<0.05);the levels of IFN-? and IL-2 after the treatment in the two group were both significantly higher than before(P<0.05);But after the treatment,the IL-4 in compound glycyrrhizin injection group was significantly lower,and IL-2?IFN-? was significantly higher,compared with the control group(P < 0.05).The compound glycyrrhizin injection group total effective rate was higher than the control group of patients(P<0.05).Conclusion:1.The degree of the Th1/Th2 immune imbalance in purpura renal injury group and the purpura abdominal type group were significantly higher than simple purpura group,which indicated that the Th1/Th2 immune imbalance is relevant to the renal injury and the gastrointestinal injury.It can be regarded as an important indicator to estimate the prognosis of patients with allergic purpura.2.Compound glycyrrhizin injection has siginificant immunoloregulation on HSP patients,and plays its role through regulating the balance of Th1/Th2 probably.Compound glycyrrhizin injection can improve the clinical effects of treatment,it is worth popularization and application.
