Objectives:Retrospective analyze the endoscopic and clinicopathological characteristic and treatment of the colorectal diminutive polyps(DP)patients who were undergoing colonoscopy in Yan'an University Affiliated Hospital from July 2016 to July 2017,with a view to provide a theoretical basis for its diagnosis and follow-up strategy.Methods: Collected the data of the patients with colonoscopy at the Yan'an University Affiliated Hospital from June 2016 to December 2017,included those who met the DP diagnostic criteria(?5 mm in colorectal polyps),excluded patients with incomplete clinical pathology and larger polyps(> 6mm colorectal polyps),recorded and analyzed the general data,polyps size,location,morphology,histopathological examination results and endoscopic treatment of the DP patients,the SPSS20.0 software package was used for statistical processing.Results:Objectives:Retrospective analyze the endoscopic and clinicopathological characteristic and treatment of the colorectal diminutive polyps(DP)patients who were undergoing colonoscopy in Yan'an University Affiliated Hospital from July 2016 to July 2017,with a view to provide a theoretical basis for its diagnosis and follow-up strategy.Methods: Collected the data of the patients with colonoscopy at the Yan'an University Affiliated Hospital from June 2016 to December 2017,recorded and analyzed the general data,polyps size,location,morphology,histopathological examination results and endoscopic treatment of the DP patients,the SPSS20.0 software package was used for statistical processing.Results:1.Relevance ratio of colorectal diminutive polyps: Collected the data of the patients with colonoscopy at the Yan'an University Affiliated Hospital from January 2016 to December 2017,total 5373 patients were enrolled in colonoscopy,1467patients(27.3%)were diagnose intestinal polyps through the endoscopic and pathological.There were 361 patients with 506 cases meet the diagnostic criteria of colorectal diminutive polyps and the information are intact,which accounting for about 6.7% of colonoscopy over the same period,24.6% of the cases of intestinal polyps detected in the same period.2.The sex and age distribution of patients with colorectal diminutive polyps: selected 361 patients of colorectal DP patients,209 males and 152 females,the male to female ratio was 1.4: 1;There were 312 male and 194 female in this colorectal DP group,the ratio of men and women is 1.6: 1.The age are between 25 to 85 years old,with an average age of 54.3 years.3.The endoscopic features and histopathological features of colorectal diminutive polyps:264 patients have single polyp(73.1%),multiple polyps in 97 patients(26.9%)? The colorectal DP of?3 mm are 120(23.7%)cases,and 386(76.3%)cases>3mm..In this study,incolorectal polyps in the sigmoid colon and rectum is higher.According to pathological results,140(27.7%)inflammatory polyps,59(11.7%)hyperplastic polyps,283(55.9%)colorectal adenomatous polyps,24 cases(4.7%)accompanied by low level intraepithelial neoplasia.Advanced adenoma was detected in 7(1.4%),not detected the adenocarcinoma.Adenomatous polyps are more likely to be associated with lower grade intraepithelial neoplasia than non-adenomatous polyps(P <0.01).4.The relationship between sex,age,endoscopic characteristics and histopathological features in patients with colorectal DP: Male and female single polyps and adenomatous polyps were more common,but the detection rate of multiple polyps and adenomatous polyps was higher in male than in females(P<0.01).In all age groups,sessile polyps and adenomatous polyps were more common,but the detection rate of pedunculated polyps and adenomatous polyps in the old groups was higher than that in the young group(P<0.05).5.Colorectal DP endoscopic characteristics and histopathological features,microscopy treatment: The detection rate of the adenoma polyp in the colorectal DP of> 3mm is higher than ?3mm.The detection rate of adenomatous polyps in pedunculated polyps was higher than that of sessile polyps.In this study,506 colorectal DP were mainly removed by biopsy forceps,with 383(75.7%),coagulation 43(8.5%),electrocoagulation 13(2.6%),and EMR 1(0.2 %),66 unprocessed(11.3%).In this study,the colorectal DP were mainly removed by forceps,but the single,>3mm and pedunculated polyps have a higher rate of coagulation /electrocoagulation/EMR.Conclusion:The statistical analysis of 506 colorectal diminutive polyps at the in Yan'anUniversity Affiliated Hospital showed that the detection rate of diminutive polyps in male colorectal was higher than that in females,and the detection rate was higher in middle-aged and old-aged patients in different age groups.The detection rate of adenomatous polyps in various pathological types of colorectal DP was 283(55.9%),and lower grade intraepithelial neoplasia was more likely to occur than non-adenomatous polyps.The detection rate of male adenomatous polyps was higher,and the detection rate of adenomatous polyps in the old groups was higher than that of the young group.Colorectal DP site is more common in rectum and sigmoid colon.The proportion of pedunculated polyps in the old age groups was higher than that of the young group,and the detection rate of >3 mm and pedunculated adenoma polyps was high.The endoscopic treatment of colorectal DP was performed with biopsy forceps as the main method,but single,>3mm and pedunculated polyps were treated with coagulation /electrocoagulation/EMR with more than ?3mm and a large proportion of sessile polyps.
