Chapter one The Diagnostic Accuracy of Auhua CBI System on Colorectal Diminutive polypsObjective: To investigate whether the accuracy of NICE classification for the colorectal diminutive polyp under the domestic endoscopic CBI technique is enough and whether the clinical application of different techniques is stable and suitable for the promotion and application of PIVI strategy.Methods: From June 2014 to April 2015,three physicians with different qualifications were enrolled in this study.239 patients,248 patients and 248 patients were enrolled by three physicians.A total of 707 polyps were included in the study.Read the color,blood vessels and the surface structure and determine the NICE classification to predict the nature of polyps.To pathology as the gold standard,the CBI system for the diagnostic accuracy of colorectal diminutive polyps,compared with the accuracy of endoscopic diagnosis of patients with or without differences.Results: The diagnostic sensitivity,specificity,positive predictive value,negative predictive value and accuracy of Auhua CBI system using NICE classification was 79.3%(74.2%-83.7%),86.5%(82.7%-89.6%),81.2%(76.2%-85.4%),85.0%(81.1%-88.2%)and 83.5% for colorectal diminutive polyps.The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of three physicians in diagnosing colorectal diminutive polyps were 85.9%,88.5%,84.9%,89.3% and 87.4%,respectively;77.4%,89.9% and 84.5 %,84.7% and 84.7%;76.2%,82.2%,76.2%,82.2% and 79.6%;p values were 0.21,0.12,0.19,0.28,0.07 respectively.Conclusion: 1.The diagnostic accuracy of the domestic endoscopic CBI system in distinguishing neoplastic colorectal diminutive polyps from non-neoplastic ones is of higher accuracy;2.Different qualificated physicians of the accuracy of the diagnosis of colorectal polyps no significant difference in clinical application;3.CBI system of colorectal negative predictive value of 85.0%,has not yet reached the PIVI strategy,but in clinical application has a certain guiding significance.Chapter two A Parallel Randomized Controlled Study of Auhua CBI System and Olympus NBI System in the Diagnosis of Colorectal diminutive polypsObjective: To investigate whether the accuracy between Auhua CBI and Olympus NBI in judging the nature of colorectal polyposis tumors is quite similar.Methods:From May 2015 to May 2016,two physicians with the same qualifications were randomized to use CBI or NBI to observe diminutive polyps.NICE classification was recorded and compared with pathology.A total of 566 patients were enrolled and 612 polyps were evaluatedResults:The diagnostic sensitivity,specificity,positive predictive value,negative predictive value,and accuracy of the CBI for colorectal diminutive polyps were 74.8%,83.3%,74.1%,83.8%,80.0% The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of the NBI were 86.1%,81.5%,89.3%,84.1%,87.4%,respectively.Except for the positive predictive value,the remaining p values were all bigger than 0.05.Conclusion :1.The sensitivity,specificity,accuracy and negative predictive value of CBI and NBI in the diagnosis of colorectal polyps are not statistically different,and the diagnostic level is comparable.2.The negative predicted value of CBI in the diagnosis of colorectal diminutive polyps was 83.8% and the NBI was 87.4%,all of which did not meet the PIVI strategy.
