Objective1.To know about the real experience of nurses while making decision of having a second child2.To explore situation of nurse's decisional conflict of having a second child,work-family conflict and social support3.To explore factors of nurse's decisional conflict on having a second child Methods1.Qualitative research: A qualitative study of nurse's fertility decisionSemi-structured interviews were conducted with 13 nurses who have had at least one child.Data were analyzed by Colaizzi's method.2.Quantitative research: Situation and factors associated with nurse's decisional conflict on having a second child(1)Validation of Decisional Conflict Scale: Decisional Conflict Scale was validated through Brislin Translation Method,Chinese version of Decisional Conflict Scale was conducted through translation and back-translation,cultural adjustment,pilot study and validity and reliability test,with good validity and reliability(Cronbach's ? =0.876).(2)The Chinese version of Decisional Conflict Scale(DCS)was used to evaluate nurse's decisional conflict on having a second child,general information questionnaire,Work-Family Conflict Scale(WFC)and Perceived Social Support Scale(PSSS)were conducted among nurses who have had at least one child in Guangzhou.Data were analyzed using SPSS21.0 software.Statistical methods including descriptive statistics,one-way analysis,correlation analysis,and multiple linear regression analysis were used.Results1.Qualitative researchThe experience of nurses on making the decision of having a second child could be defined into 4 themes,including low fertility desire,many influencing factors,work influence family badly and family support made a difference.2.Quantitative research2.1 Total score of work-family conflict was(3.10±0.84),score of work-family conflict was(3.46±0.91),and family-work conflict(2.73±0.95),total score of decisional conflict was(40.9±14.8),and for social support was(57.85±15.77).2.2 According to the second-child plan,207(29.1%)had planned to have a second child and 133(18.7%),116(16.3%)have already had a second child,255(35.8%)did not want to give birth to a second child,81(11.4%)didn't have the plan now and 52(7.3%)didn't make the decision.2.3 Correlation analysis results showed that gender,age and income were negatively correlated to nurse's decisional conflict on having a second child(r=-0.092~-0.105,P<0.05),marriage situation,employment type,working position and age of the first child were positively correlated to nurse's decisional conflict on having a second child(r=0.099~0.147,P<0.05),scores of work-family conflict was positively correlated to nurse's decisional conflict on having a second child(r=0.145~0.315,P<0.05),scores of social support were negatively correlated to nurse's decisional conflict on having a second child(r=-0.143~-.231,P<0.05).2.4 Results of one-way analysis showed that There were significant difference of Decisional conflict of having a second child among demographics(gender,age,marriage,employment type,working position,income,medical insurance,age of first child and second child plan).Multiple regression showed that gender,employment type,time-based work-family conflict,behavior-based work family conflict and family support were major factors of nurse's decisional conflict on having a second child(B=-5.750~3.266,P<0.05).Conclusion1.Nurse's desire of having a second child was low,associated factors including education,raising spending,income,medical insurance,time-based work-family conflict,family pressure and family support.2.Nurse's decisional conflict on having a second child was high and they felt medium to high level work-family conflict.Nurse's decisional conflict on having a second child was positively correlated to employment type,time-based work-family conflict,behavior-based work family conflict and was negatively correlated to gender and family support.3.Managers should know about nurses' fertility needs,care about nurses and adapt human resource,approve applications of taking care of their family,increase more nurses,organize training to help nurses adapt to their work after maternity leave,encourage nurses to ask for help so as to help them balance their work and family,increase job-related and family-related happiness,which would turnover rate of nurses and maintain a stable team of nursing professionals.
