ObjectiveIn this study,we use the digital image fusion to build a three-dimensional facial soft tissue measurement model, establish a fusion model of CT data and 3D scanning images, and then make a three-dimensional measurement of soft tissue changes before and after orthognathic surgery for patients with unilateral jaw deformities. Objective to explore the change rule of facial soft tissue in patients with unilateral jaw deformity after operation, and to clarify the change of facial symmetry and the change direction of three signs in each landmark,so as to provide guidance for the design of operation plan.MethodsFrom October 2016 to October 2017,three dimensional scan images of patients with unilateral mandibular deformities in orthodontic and orthognathic surgery in our hospital were collected.Part I: using digital image fusion to establish the three-dimensional measurement model of facial soft tissue. The rapid and simple maxillofacial soft tissue three-dimensional digital fusion model of facial soft tissue to establish in June before and after the operation,fixed,three-dimensional image data of soft tissue were measured, and three-dimensional analysis of maxillofacial soft tissue changes in patients with sudden partial jaw deformity before and after operation.Part II: the three-dimensional measurement of the soft tissue changes before and after the orthognathic operation in the patients with malformation. On the basis of the 3D fusion model obtained from Experiment 1,we first analyze the symmetry change of each landmark, and then analyze the coordinate changes in three dimensions respectively.Statistical analysis: in SPSS19.0 software, paired t test was used for preoperative and postoperative three-dimensional coordinate changes(P < 0.05)difference was statistically significant.Result:There was a statistically significant difference in the asymmetry coefficient between the mental anterior points, chin vertex, submental points, bilateral alar base points and bilateral buccal points before and after surgery(P<0.05).With the front view of the coordinate changes were statistically significant except the labrale superius outside of the selection marker in horizontal direction, and the gnathion and gnathion, basis of bilateral alar changes significant to illustrate its surface under 1/3 symmetry is improved, the marking points correction degree also showed a trend of overall change gradually from top to bottom the visible;sagittal maxillary advancement, mandibular setback and vertical decline trend;the visible trend of maxillary mandibular elevation. Conclusion:This experiment proved that orthognathic surgery can effectively improve the symmetry of lateroprognathism deformity in patients with facial soft tissue, and improve facial maxillary retrusion of mandibular prognathism, through bimaxillary surgery can significantly correct lateroprognathism deformity, and can achieve better symmetry by contour dressing, has guiding significance for rival art design.
