Objective:To investigate the pathogenesis,possibility causes and treatment strategies of hypernatremia and hyperchloremia in patients with extensive burns.Methods:A total of 6 patients with severe sodium burn(TBSA ? 50%)complicated with hypernatremia were selected from 2016 to 2017 in the undergraduate department,aiming at the characteristics of onset of high sodium and hyperchloremia,their causes,treatment measures and prognosis.Conduct research and discussion.General clinical data during hospitalization,serum sodium,time of hypernatremia,duration of hypernatremia,blood chlorine,and blood glucose were collected for retrospective analysis.The data is analyzed and processed using SPSS19.0 software.Results:1.The incidence of hypernatremia and hyperchloremia in patients with extensive burns in this ward over the past two years was high(22.22% vs 10.16%-20.83%),occurred early(day 3 after the earliest injury),and lasted for a long time(For a maximum of 15 days,the increase is high(blood sodium values up to 178.6 mmol/L and blood chlorine values up to 130.2 mmol/L).2.The causes of hypernatremia and hyperchloremia in the early stage of severe burn patients.In addition to recognized factors,the use of high temperature climate,early suspended beds and far infrared rays in South China may also be an important factor.3.The prognosis of patients with extensive burns and the level of serum sodium,duration of hypernatremia,and time of occurrence,but also with the burn area and blood glucose levels;and when the burn area is larger,hypernatremia occurs earlier,duration Longer-term patients had poorer prognosis and higher mortality(sodion t=5.59,P<0.05,chloridion t=2.156,P<0.05);patients' sodium ion concentration and chloride ion concentration affect large-area burn patients Independent risk factors for death(sodion hazard ratio 4.507,P<0.05,chloridion hazard ratio 4.269,P<0.05;patient's blood glucose level may have a certain correlation with patient prognosis(glucose risk ratio 4.760,P<0.05),But further research is needed.4.Comprehensive treatment measures such as sodium restriction,chlorine restriction,oral administration,and gastric tube supplementation with sufficient water,sodium diuresis,and hemodialysis can effectively reverse hypernatremia and hyperchloremia(6/6).Conclusions:1.The incidence of extensive burns complicated with high sodium and hyperchloremia is high,occurring early,and lasting longer,with a higher increase.2.Timely and effective clinical treatment can completely reverse the hypernatremia and hyperchloremia.3.The causes of hypernatremia and hyperchloremia Besides the recognized factors,the use of high temperature climate,early suspended beds and far infrared rays in South China may also be an important reason.
