Background:Postherpetic neuralgia is a common neuropathic pain.The pain of pain is mainly a sense of acupuncture,burning sensation,knife cut,etc.,Postherpetic neuralgia has a more severe degree of pain,the treatment process is longer and the general clinical prognosis is poor.Therefore,postherpetic neuralgia seriously affects the quality of life of patients and becomes a serious social health problem.Drug treatment is the main method of clinical treatment of postherpetic neuralgia,but for the treatment of refractory postherpetic neuralgia is not ideal,and long-term use of drugs can cause huge side effects,damage the patient's physical health,so more and more research is more inclined to use interventional therapy,such as physical therapy such as pulsed radiofrequency therapy and electrical stimulation therapy,or nerve block therapy.Nerve block is directly through the drug intervention or physical stimulation of the intervention in the way the nerve tissue,thereby affecting the nerve conduction to impede the effect of nerve conduction.General nerve blockade in the peripheral nerve,ganglia and other nerve tissue near.Treatment of nerve block therapy can be achieved by preventing the pain signal transmission to achieve the purpose of relieving neuropathic pain.The current study suggests that nerve block treatment of patients with neuropathic effect is ideal,can reduce the patient's neuropathic pain,and promote herpes healing,thereby shortening the course of disease and shorten the cure time.Recent studies have found that the clinical use of pulsed radiofrequency treatment of neuropathic pain have a better therapeutic effect.However,the current part of the clinical treatment is still using a combination of drug and nerve block treatment of treatment of postherpetic neuralgia,so compared to pulsed radiofrequency and nerve tissue on the treatment of postherpetic neuralgia,screening for better treatment Means,very clinical significance.The purpose and meaning:To observe the efficacy of pulsed radiofrequency ablation and nerve block in patients with postherpetic neuralgia,and to provide a reference for the effective treatment of refractory postherpetic neuralgia and to explore and compare the effects of pulsed radiofrequency and nerve block therapy Effectiveness and security.Methods:90 patients with refractory postherpetic neuralgia were randomly divided into two groups,among them,45 cases of nerve block group(control group),will be based on conventional drug therapy,nerve block treatment;pulsed radiofrequency group(study group)45 cases,based on conventional drug therapy,pulsed radiofrequency treatment.1.The age,sex,duration of illness,comorbidities,and before treatment,resting VAS score,onset VAS score,attack frequency,duration and other basic information.2.3 days after treatment,1 week after treatment and 6 months after treatment,resting VAS score,onset VAS score,onset frequency,duration,etc.3.The efficacy and satisfaction of the patients after treatment in June.4.The occurrence of adverse events and its safety were discussed.Results:1.There was no significant difference in the VAS score between the pulsed radiofrequency group and the nerve block group before treatment(P> 0.05).The VAS score of the pulsed radiofrequency group was 3 weeks after treatment,1 week after treatment and 6months after treatment(P <0.05).There was significant difference between the three groups after treatment(P <0.05).The VAS score of the nerve block group was significantly higher than that before treatment(P <0.05)The There was also a significant difference between the two groups in the VAS score(P <0.05)at 3 days after treatment,1 week after treatment and 6months after treatmentThe effective rate was 93.3% in the pulsed radiofrequency group and73.3% in the nerve block group,the difference was statistically significant(P <0.05).2.The effective rate was 93.3% in the hematoma occurred in the two groups.4.The satisfaction rate was 97.5% in the pulsed radiofrequency group and 78.8% in the patients with the nerve block group.The satisfaction rate of the patients in the pulsed radiofrequency group was significantly higher than that in the nerve block group(P <0.05).Conclusion:(1)The study group and the God control group can effectively relieve refractory postherpetic neuralgia,which pulse group of pain relief than the control group of pain relief is more obvious.(2)The study group in the treatment of refractory postherpetic neuralgia in the control group has a significant therapeutic advantage.(3)The study group and the control group in the treatment of refractory postherpetic neuralgia has a strong safety.(4)The study group in the treatment of refractory postherpetic neuralgia in patients with satisfaction was significantly higher than the control group.Our study found that pulsed radiofrequency ablation and nerve block in the treatment of refractory postherpetic neuralgia effect is ideal,in which pulsed radiofrequency has a more obvious therapeutic advantage,worthy of further study and actively applied to clinical.
