BackgroundCoronary atherosclerotic heart disease(CHD)and its related complications have been the most high risk disease threatening to human health.Although it has made great progress in diagnosis,prognosis and treatment,but it is still the major leading cause of death in China.miRNA is a small non-coding RNA molecule,that functions in post-transcriptional regulation of gene expression,and it expression signature patterns that are associated with pathological CHD in humans have been identified.miRNA is a new prognostic biomarker for the early identification of CHD,ACS,and AMI patients.The therapy based on miRNA may be a promising therapy for the cardiovascular diseases.miRNA-137 is a kind of miRNAs.It has been implicated to act as a tumor suppressor in several cancer types,and may promoted the progression of cardiovascular diseases through a variety of signaling pathway.ObjectivesAIM to investigate the different expression and the clinical significance of miRNA-137 between the patients with acute coronary syndrome and the subjects without coronary artery lesions.MethodsWe collected the plasma of ACS patients(n=68 cases)and those patients without coronary artery lesions(n=32 cases).The patients with ACS were divided into two subgroups(AMI group and UA group)according to electrocardiogram(ECG)and myocardial enzymes.The plasma miRNA-137 expression was tested by qRT-PCR in different group.The level of CK-MB,cTnI,proBNP,SYNTAX score and LVEF of the patients were also recorded.The Spearman correlation analysis was performed between miRNA and CK-MB,cTnI,proBNP,SYNTAX score and LVEF.Results1.The plasma miRNA-137 levels of ACS were significantly higher than that of the control group [(32.11±1.39)vs(31.03±1.04),P<0.05].The plasma miRNA-137 levels of UA were significantly higher than that of the control group [(32.05±1.43)vs(31.03±1.04),P<0.05].The plasma miRNA-137 levels of AMI were significantly higher than that of the control group [(32.46±1.10)vs(31.03±1.04),P<0.05].There were no statistical difference between the AMI and UA [(32.05±1.43)vs(32.46±1.10),P=0.365].2.Spearman correlation analysis showed that miRNA-137 had a positive correlation with proBNP(r=0.218,P<0.05)and cTnI(r=0.249,P<0.05).3.There were no statistical difference among the different vessels group of miRNA-137 [(32.21±1.45)vs(31.71±1.16)vs(32.34±1.47),P=0.350].4.The level of plasma miRNA-137 had statistical significance between the Non-CHD group,syntax low score group,syntax intermediate score group,and syntax high score group [(31.03±1.04)vs(32.10±1.41)vs(32.22±1.38)vs(31.97±1.47),P<0.05].The plasma miRNA-137 levels of low score and intermediate score were significantly higher than that of Non-CHD.However,there were no statistical difference between high score group and other groups.5.Multivariate logistic regression analysis showed that miRNA-137 was the independent predictors of ACS.6.Receiver operator curves(ROC)for miRNA-137 for prediction of ACS showed the areas under the curves(AUC)were 75.0%(P<0.05).When the expression amount of miRNA-137 was 30.88,the sensitivity and specificity were 82.4% and 65.6%.cTnI for prediction of AMI showed the AUC were 87.3%(P<0.05).When the expression amount of cTnI was 0.0325ng/ml,the sensitivity and specificity were 80.0% and 90.0%.The AUC of the expression of miRNA-137 was 68.2%(P=0.06).Conclusions1.The miRNA-137 expression in patients with ACS was significant higher than that in the control group.2.The miRNA-137 was positively correlated with proBNP and cTnI.3.The miRNA-137 expression was significantly different between different SYNTAX evaluation groups,which may partly reflect the complexity of vascular lesions.4.The miRNA-137 might be the independent risk factors of ACS and the sensitivity and specificity of miRNA-137 were high,as well as which in certain degree could predict the severity of ACS.It maybe a potential biomaker for the early diagnosis of ACS.In the early diagnosis of AMI,miRNA-137 was worse than cTnI.
