| Objective: According to the randomized controlled study,the clinical effect of Jingxuan Ning decoction on cervical spondylosis of vertebral artery type(Stagnation of phlegm and blood stasis)and the effect on vertigo symptom function and vertebrobasilar artery hemodynamics were observed.Methods:From January 2017 to December 2017,60 patients with vertebral artery type cervical spondylosis(phlegm and blood stasis obstruction)were selected from the diagnosis and treatment center of the second affiliated Hospital of Henan University of Traditional Chinese Medicine.Sixty patients who met the diagnostic criteria were divided into two groups according to random digital table: the experimental group(30cases)and the control group(30 cases).The experimental group was treated with Zhang Jianfu experience of removing phlegm and removing blood stasis with Jingxuanning decoction,one dose per day,one in the morning and one in the morning,half a time.And the control group was given Betastine Mesylate(Betahistine),three times a day,one at a time,take warm boiled water 30 minutes after three meals.And both groups were treated with reposition manipulation once a week for 4 weeks.Before and after the treatment,the patients in the two groups must ensure the authenticity of the score according to the VAS score standard of TCM syndromes and the score standard of vertigo symptom function,and according to their own actual situation.The teachers in the color doppler room of our hospital performed transcranial doppler ultrasound examination to record the blood flow velocity of each artery in detail,and then compared the vertigo score,the total score of vertigo symptom function,the TCM syndrome score and the VAS score between the two groups before and after treatment.The contractile phase and mean blood flow velocity of basilar artery and left and right vertebral artery were examined by transcranial doppler(TCD).The patients were followed up for 2 weeks and 3 months after treatment.The early and long term effects of the two treatments were evaluatedaccording to the score of vertigo symptom function.Results:1 TCM syndrome scores,VAS scores,dizziness scores,total functional scores of vertigo symptoms,systolic blood flow velocity(Vs),and mean blood flow velocity(Vm)in the basilar and left and right vertebral arteries before and after treatment Comparison(P<0.05)was statistically significant,indicating that both treatments are effective;2 After the end of the treatment,the total scores of vertigo,vertigo symptoms,VAS scores,and TCM syndrome scores were statistically significant(P<0.05).3 Before and after treatment,the systolic phase and mean blood flow velocity of basilar artery,left and right vertebral artery were compared between the two groups(P>0.05),and there was no statistical significance.4 After 2 weeks of treatment and 3 months after the end of treatment,the vertigo scores and the total scores of vertigo symptoms in the two groups were compared(P<0.05).5 Before and after treatment,2 weeks,after the end of treatment and 3 months after the end of treatment,the total scores of the vertigo and vertigo symptoms in the two groups of patients were analyzed by repeated measures analysis of variance.The time factor P=0.000,indicating that the two groups were at their respective time points.The vertigo score was statistically significant compared with the total score of vertigo symptoms;the interaction between time and group was P<0.05.The results showed that the two groups had statistically significant differences in the time-varying trends of the total scores of vertigo scores and vertigo symptoms;6 The effective rate in the trial group was 93.33%,and the effective rate in the control group was 80%.Compared with the results after the rank sum test,(P<0.05),the clinical efficacy between the two groups was statistically significant;7 During the course of treatment and after the end of treatment,there was no adverse reaction in both groups;there was no statistically significant difference in AST,ALTand Cr between the two groups before and after treatment(P>0.05).Conclusion:1 The clinical curative effect of Jingxuanning decoction in treating cervical spondylosis of vertebral artery type is better than that of betahistine,it can obviously improve vertigo symptom,work ability of daily life,ability of psychological and social adaptation,reduce syndrome integral of traditional Chinese medicine,relievepain of head and neck;speed up the vertebrobasilar artery contraction and mean blood flow speed,improve the role of cerebral blood supply.2 The effect ofJingxuanning decoction in improving vertigo symptoms and vertigo symptoms is relatively slow,but the long-term effect is more stable and better than that of betahistine,and it has the characteristics of safe and effective.3 In improving the blood flow velocity of vertebrobasilar artery,Jingxuanning decoction and betahistine effect equal. |
