Clinical Study On Treatment Of Cervical Spondylotic Myelopathy By Anterior Cervical Decompression,bone Graft Fusion And Internal Fixation Combined With Yugu Capsule

Objective: To observe the clinical efficacy of ACDF combined Yugu capsule in the treatment of cervical spondylotic myelopathy.To study the effect of Yugu capsule in relieving cervical spine dysfunction,restoring spinal cord function,changing cervical curvature,and promoting interbody fusion after ACDF.Methods: Patients who were admitted to Zhengzhou Orthopaedic Hospital from July 2016 to September 2017 and were diagnosed with cervical spondylotic myelopathy were studied.According to the inclusion criteria,a total of 60 patients were collected,according to the order of patient visits sequentially numbered,randomized according to the random number table method: 1 test group: ACDF + Yugu capsule + conventional treatment,a total of 30 cases;2 control group : ACDF + conventional treatment,a total of 30 cases.Statistical analysis of the two groups of patients in the JOA score,cervical dysfunction index(NDI),cervical curvature,fusion rate of interbody fusion and other aspects.Results: 1.The differences between the two groups in terms of general con ditions(gender,age,duration of disease),operation time,intraoperative blood loss,surgical methods,surgical segments,and preoperative JOA scores,NDI,cervical curvature,clinical symptom scores,etc.No statistical significance(P>0.05).2.JOA score: Postoperative JOA scores in both groups were significantly higher than those in the same group.There was no significant difference in JOA scores between the two groups at 1 week,3 months,and 6 months postoperatively(P>0.05).3.Cervical Dysfunction Index(NDI): The postoperative NDI of both groups was significantly lower than that of the same group before surgery.There was no significant difference in NDI between the two groups at 1 week and 6 months postoperatively(P>0.05).The difference in NDI between the two groups at 3 months postoperative was statistically significant(P<0.05).4.Cervical curvature: The postoperative cervical curvature was significantly higher in both groups than in the same group.There was no significant difference in cerv ical curvature between the two groups at 1 week,3 months after operation,and 6 months after operation(P>0.05).5.Chinese medicine clinical symptom scores: Postoperative chinese medicine clinical symptom scores of both groups were significantly lower than those of the same group before surgery.There was no significant difference in chinese medicine clinical symptom scores between the two groups(P>0.05).The chinese medicine clinical symptom scores of the two groups at 3 months and 6 months were statistically significant(P<0.05).6.Fusion rate of interbody fusion: There was no significant difference in fusion rate between the two groups at 3 months and 6 months after operation(P> 0.05).Conclusion: Anterior cervical decompression,bone graft fusion and internal fixation combined with Yugu Capsule can quickly relieve the preoperative spinal cord compression in patients with CSM,and provide necessary conditions for the recovery of spinal cord function.It can effectively relieve the patient's cervical spine dysfunction,change the patient's physiological curvature of the cervical spine,improve the patient's chinese medicine clinical symptom scores,and improve the patient's quality of life.Yugu capsule has the effect of relieving cervical spine dysfunction,improving chinese medicine clinical symptom scores and improving quality of life after ACDF surgery.It also has some effects on accelerating bone graft fusion,but it can not significantly promote the fusion of intervertebral bone graft,change the physiological curvature of the cervical spine and promot e the recovery of spinal cord function.ACDF combined with Yugu Capsule for the treatment of CSM is a feasible treatment plan of integrated Chinese and Western medicine,which is worthy of clinical application.
