Part I:Efficacy comparison of ROI-CTM and traditional fused titanium plate in ACDF.Objective:to study the different curative effects of ROI-CTM and traditional fused titanium plate in ACDF.Methods:through retrospective study,January 2010 to December 2016,Sixty patients undergoing ACDF surgery were selected In the orthopedics inpatient department of our hospital.Group A:30 patients,using traditional fusion?autologous iliac bone graft?titanium plate internal fixation;Group B:30 patients,were fixed by ROI-CTM self-locking anterior intervertebral fusion system?autologous iliac bone graft?.Two groups of patients were evaluated:preoperative conditions?age,gender,segmental distribution,VAS score,JOA score,Cobb Angle,Kellgren classification?;Intraoperative situation?surgicaltime,bleeding?;Postoperative conditions?complications,VAS score,JOA score,Cobb Angle,fusion situation,Odom grading,Kellgren classification?.SPSS 22.0 statistical software was used for statistical analysis.Results:?1?preoperative situation:There was no significant difference in general data and surgical segment distribution between groups A and B?P>0.05?.?2?intraoperative situation:the intraoperative time and intraoperative blood loss of group B were significantly lower than that in group A?P<0.05?.?3?postoperative conditions:in group A,2 patients?6.67%?had difficulty swallowing,and 3 cases?10.00%?had hematoma in the neck,while none of the above complications occurred in group B?P<0.05?.In the two groups,patients'VAS score and JOA score at"12months after surgery"were significantly better than"one month after surgery"?P<0.05?,A?80.0%?and B?83.6%?was similar to that of JOA score?P>0.05?.After 12 months,all the patients had bone fusion.Odom grading:The success rate of group B?90.00%?was higher than group A?86.67%??P<0.05?.Kellgren classification:The incidence of degeneration in group A?23.33%?was significantly higher than group B?16.67%??P<0.05?.Conclusion:It has similar clinical effect of ROI-CTM and traditional fusion titanium plate in ACDF.Because the former is easy to operate and has the advantages of design,the patients have less intraoperative bleeding,shorter operation time,less postoperative complications,and a low incidence of long-term adjacent segmental degeneration.Part?:Efficacy of ROI-CTM and Zero-P in postoperative complications such as dysphagia and neck hematoma after operation in the two groups.After the postoperative review,the evaluation data were significantly improved compared with the preoperative period?P<0.05?,and there was no statistically significant difference between the groups?P>0.05?.Conclusion:clinical efficacy and effect on adjacent segmental degeneration in ACDF,ROI-C and Zero-P have similar effects.However,compared with the Zero-P,the ROI-CTMreduces the removal of periosteum during the operation,and does not require drilling or screwing,so the operation time is shorter.Part?:The curative effect of OICPC and autologous bone in ACDF.Objective:to study the different curative effects of ROI-CTM fusion internal fixation system using OICPC or autologous bone in ACDF.Methods:through retrospective study,January 2010 to December 2016,forty patients undergoing ACDF surgery were selected In the orthopedics inpatient department of our hospital.Group OICPC:20 patients,using ROI-CTM?OICPC graft?;Group autologous bone:20 patients,using ROI-CTM?autologous iliac bone graft?.Two groups of patients were evaluated:Preoperative condition?patient age,gender,distribution of operative segment?;Postoperative conditions?complications,rejection,fusion situation,Odom classification?.SPSS 22.0 statistical software was used for statistical analysis.Results:?1?preoperative situation:there was no significant difference in general data and surgical segment distribution between two groups.?2?postoperative conditions:In group autologous bone,5 cases?20%?of hematoma,0 case?0%?of infection,and 20cases?100%?of chronic pain were found in the iliac bone graft area after surgery,and the patients were relieved within 2 weeks after positive change and treatment.In group OICPC:there was no need for autologous iliac bone graft,so there are no complications like these?P<0.05?.There was no statistically significant difference in the number of fusion cases,fusion rate,Odom grading and success rate between the two groups at each time point?P>0.05?.Conclusion:OICPC can be used in ACDF surgery as better bone graft material.Secondary trauma and related complications were avoided when you take the bone from the autologous bone.In addition,the biocompatibility is high,the long-term fusion rate is close to the autologous bone,and has good mechanical support and osteogenesis characteristics.
