Objective:By observing the short-term clinical effect of Yugu Capsule combined with MIS-TLIF in the treatment of degenerative lumbar spinal stenosis, the effect of traditional Chinese medicine is explored, which provides a more effective method for accelerating postoperative recovery and the combination of traditional Chinese and western medicine in treating degenerative lumbar spinal stenosis. Methods:The study subjects were patients with degenerative lumbar spinal stenosis who met the inclusion and exclusion criteria of this study and were hospitalized in the spine orthopedics department of Zhengzhou Orthopaedic Hospital from September2018 to August 2019 and planned to undergo surgical treatment.A total of 62 patients were included in the study, signed the informed consent, and randomly divided into experimental group and control group by envelope method, with 31 cases in each group.Patients in both groups were given MIS-TLIF operation. The experimental group was given conventional treatment and oral Yugu capsule after operation. The control group was given routine treatment only after operation. All patients were followed up for at least 6 months, and the VAS score of lumbar and leg pain,ODI score of lumbar spine were recorded in two groups of cases preoperative,1 month,3months,and 6 months after operation.Intervertebral bone graft fusion was recorded with reference to Birdwell standard at 3 and 6 months after operation. The effect of modified Mac Nab was evaluated at the last follow-up.SPSS 20.0 software was used for data analysis, and Graph Pad Prism 5.0 software was used to draw statistical graphs.Results:1.A total of 62 patients were included, including 31 in the experimental group and 31 in the control group.Eventually, one patient fell off,and the remaining patients were followed up as required.2.Comparison of baseline data(gender, age, course of disease, responsible segment, etc)between the two groups was not statistically significant(P>0.05),and was comparable.3.Scoring status: In terms of the VAS score of lumbar and leg pain,ODI score of lumbar spine, the comparison within each group gradually decreased after operation, and the difference was statistically significant(P<0.05).The above scores were not statistically different between the two groups at preoperative and 6 months after operation(P>0.05).The scores of the experimental group at 1 month and 3 months after operation were lower than those of the control group, and the differences were statistically significant(P<0.05).4.Intervertebral bone graft fusion: At 3 months after operation, the experimental group: 23 cases of grade I,6 cases of grade II,1 case of grade III and 1 case of grade IV,and the control group: 12 cases of grade I,9 cases of grade II,6 cases of grade III,and 3 cases of grade IV.The difference was statistically significant(P<0.05).The fusion rate was 93.55% in the experimental group and 70.00% in the control group, and the difference was statistically significant(P<0.05).At 6 months after operation, the experimental group: 26 cases of grade I,4 cases of grade II,1 case of grade III,0case of grade IV,and the control group: 20 cases of grade I,7 cases of grade II,2cases of grade III and 1 case of grade IV.The difference was not statistically significant(P>0.05);the fusion rate was 96.77% in the experimental group and90.00% in the control group, and the difference was not statistically significant(P>0.05).5.Modified Mac Nab Efficacy Evaluation: At the last follow-up,the experimental group: 25 patients were assessed as“excellent”,4 patients were assessed as“good”,2 patients were assessed as“fair”and no patient was assessed as“poor”.The control group: 18 patients were assessed as“excellent”,8 patients were assessed as“good”,3 patients were assessed as“fair”and 1 patient was assessed as“poor”.The differences were not statistically significant(P>0.05).The rate of satisfactory outcomes: 93.55%in the experimental group and 86.67% in the control group. The difference was not statistically significant(P>0.05).Conclusion:1.Yugu capsule combined with MIS-TLIF operation and single MIS-TLIF operation for the treatment of single-segment Degenerative lumbar spinal stenosis with Qi deficiency and blood stasis can alleviate the symptoms of lumbar and leg pain and promote the recovery of lumbar spinal function in a short-term.Both methods are effective treatments.2.After MIS-TLIF,when oral Yugu Capsule is continued for 3 months,it has more advantages than simple operation in improving pain symptoms and promoting lumbar spine function recovery.3.Yugu Capsule has certain positive significance in promoting fusion of intervertebral bone grafts.Taking 3 months can get better imaging changes, suggesting that the method of integrated traditional Chinese and western medicine can promote intervertebral bone graft more effectively and accelerate the functional rehabilitation of patients more than operation alone. It is an effective treatment plan and worthy of clinical promotion.
