ObjectiveAnalyze the status quo and utilization of health resources in a city of Hunan Province during 2014-2016 from the five perspectives of medical and health institutions,health workers,bed allocation,health fund and the utilization of health resources and compare them with those of the average level of Hunan Province and the whole nation to find out the gap;in the meanwhile,assess the fairness of the health resource allocation of the city using the Gini coefficient and the Theil index and find the advantages and disadvantages of the health resource allocation of the city in order to provide practical basis and theoretical reference for the government departments to scientifically and reasonably formulate health policies and optimize health resource allocation.Methods?.Analyze each indicator of the health resource allocation in the city of Hunan Province using a general statistical analysis method and compare it with the average level of Hunan province and the whole nation to find out advantages as well as gaps and deficiencies.In the meantime,study the utilization status of the medical and health service in the city of Hunan Province.?.Adopt the Gini coefficient method and the Theil index method to analyze the fairness trend of the medical and health resources allocationsituation in the city during 2014-2016 from the two dimensions of population allocation and geographical allocation(the main health resources include number of medical and health institutions,number of beds,number of licensed(assistant)doctors and number of registered nurses)and assess the fairness level of the medical and health resources allocation in the cityResults?.The allocation of medical and health institutions: In 2016,there was a total of 5,869 medical and health institutions in the city,of which the number of hospitals,primary medical institutions,professional public health institutions and other health institutions were respectively 129,5,615,122 and3(accounting for 2.20%,95.67%,2.08% and 0.05% respectively);Compared with 2014,in 2016,38 new hospitals and 113 new primary health institutions in the city were respectively added,while 124 professional public health institutions were cut down.?.Health bed:There were 42328 beds in 2016,including 27641 in hospitals,12630 in grass-roots medical machines,1957 in public health institutions and 100 beds in other health institutions(accounting for 65.30%,29.84%,4.62% and 0.24% respectively).The number of medical institutions per thousand population in the city was increased from 4.46 to 5.81 from 2014to 2016,exceeded the national average level,but lower than the average level of Hunan and the target of "regional health plan for 13 th Five-Year".?.Health personnel: In 2016,there was a total of 47571 health personnel in the city,of which the number of health technicians,other technical personnel,management personnel and technical staff were respectively 39494,1983,2484 and 3610(accounting for 83.02%,4.17%,5.22%,7.59%respectively).there were increasing year by year.The long rate is 12.21%,the proportion of other technicians,working skills and managers is declining yearby year;the number of health personnel per 1000 population increased from5.44 in 2014 to 6.53 in 2016,but lower than the average level in Hunan and the country.?.Health expenditure allocation: the medical and health care expenditure of urban residents in 2014-2016 years(948,1148,968.3 yuan in turn)was higher than that of rural residents' health care expenditure(691.8,832,839 yuan per person),but the proportion of urban residents(5.8%,6.7% and 5.5%in turn)were lower than those of rural residents(in turn).For 9.9%,9.3%,8.5%),the income of medical and health institutions in the city increased year by year in 2014-2016 years,which were 8 billion 968 million yuan,9 billion966 million yuan and 11 billion 197 million yuan in turn.The proportion of financial aid income increased from 13.02% in 2014 to 13.57% in 2016,and the proportion of the income of science and education projects increased from0.03% in 2014 to 0.05% in 2016.The proportion dropped from 1% in 2014 to0.91% in 2016,and the proportion of medical income / business income decreased from 85.95% to 85.47% from 2014 to 2016.?.Medical service utilization status: Medical service utilization: the person times of total diagnosis and treatment,hospitalization,discharge,and hospitalized patients in a city of Hunan are increased year by year(the annual growth rate was 3.99%,9.91%,8.36%,11.84%),and the per capita hospitalization days were decreased year by year(the annual average growth rate was-1.98%),and doctors were responsible for daily diagnosis and treatment.There were no changes in the daily hospital bed(4 times,1.8 days,respectively),and lower than the average level of Hunan and the country at the same time.The use rate of medical and health institutions in the city was73.80%.The use rate of medical beds was in turn from high to low to comprehensive hospital,specialist hospital,maternity and child health care hospital(station),traditional Chinese medicine hospital,health hospital,sanatorium,the community health service institutions(accounting for 84.08%,75.08%,74.27%,74.03%,60.76%,44.44% and 40.05% respectively).?.Equity in the allocation of health resources: According to the Gini coefficient indicators,the number of health medical institutions,beds,and registered nurses in the city were in the state of highly fair,fair,highly fair,and relatively fair according to population allocation,and in the state of highly fair,relatively fair,relatively fair and unfair according to the geographical location.In 2016,the Tel index of medical health institutions,beds,practicing(Assistant)doctors and registered nurses were 0.01,0.08,0.01 and 0.12 respectively,and the measurement of the four health resources was similar to the Gini coefficient in general.Conclusions?.The per capita ownership of health resources in a city of Hunan is relatively insufficient compared with the province.The number of hospital beds per 1000 population and the number of health personnel per 1000 population has been increasing year by year,but still lower than the average level of Hunan Province and the whole nation.?.The utilization rate of bed resources in primary health care institutions is relatively lower than that in the national average level,and the utilization rate of beds in community health service institutions is far below the national level.?.In the past three years,the allocation of health resources in the city has been improving year by year,whereas its distribution is not balanced and inclined towards central urban areas.In particular,registered nurses are unfairly distributed,which mismatching population and geographical location.
