Objective:In this paper,The clinical cases of residual symptoms after benign paroxysm positional vertigo(BPPV)were treated with Qigui Tongluo oral liquid were summarized and analyzed to observe the clinical efficacy and adverse reactions. The objective evaluation of the clinical efficacy and safety of Qigui Tongluo oral liquid on the residual symptoms of qi-deficiency blood stasis type BPPV after successful reduction is a further research and clinical application and lay a solid theoretical foundation for the Qigui Tongluo oral solution. Methods:A total of 60 patients were selected for this study. From December2016 to December 2017,they were referred to the Department of Encephalopathy at the second TCM Hospital of Guangdong Province, which met the diagnostic criteria for benign paroxysmal positional vertigo and met the exclusion criteria of this study. In accordance with the syndrome differentiation of traditional Chinese medicine as qi-deficiency and blood stasis syndrome, after successful canalith repositioning procedure(CRP),there is no positional vertigo and characteristic nystagmus.Among them, there were 25 males and 35 females, all aged between from 40 to 75 years old,and the course of the disease(the time from the onset to the success of manual reduction)was within one month. The 60 patients included in the study were randomly divided into treatment group and control group with 30 cases by random number table method. The treatment group was treated with Chinese medicine Qigui Tongluo oral liquid, and the control group was treated with western medicine bethastine methotrexate tablets. Two groups of patients took 4 weeks as a course oftreatment. Two drugs were observed after two phases of treatment.Clinical efficacy. In each group, undergraduate specialists assessed the patient's physical, emotional, and functional status before and after treatment according to the Dizziness Obstruction Scale(DHI)(Chinese version),and assessed the patient's psychological anxiety status using Hamilton anxiety(HAMA).The two groups of patients were observed before and after treatment, including blood routine, routine two stools, eight liver function tests, six renal function tests, and routine electrocardiogram changes and other adverse reactions. Results:The trial study was performed strictly in accordance with the established plan.60 cases were included in the trial and no patient was shed.Comparison of Qigui Tongluo oral liquid Group and Betahistine Mesylate Tablet Group:1.Comparison of curative effects: After treatment with drugs, the scores of TCM symptoms of the two groups of patients have improved, the effective rate of Qigui Tongluo oral liquid group is 96.67%,and the effective rate of bethraustine mesylate tablet group is 76.67%,and the former The effective rate was significantly higher than the latter(p<0.05).2.After two courses of drug treatment, the HAMA scores of the two groups were significantly lower than before treatment(p<0.05);and the score of HAMA in the Qigui Tongluo Oral Liquid group was significantly lower than that of the Betahistine mesylate tablet group(p<0.05).3.After 8 weeks of drug treatment, the total scores of DHI,function, emotion, body and other subgroups in both groups decreased compared with those before treatment. The difference was statistically significant(p<0.05);The total score of DHI,function, emotion, and triathlon scores of the oral solution group were also lower than those of the betahistine mesylate tablet group(p<0.05).4.Before and after treatment, the safety indicators of the two groups of patients were within the normal range, and no obvious abnormalities were observed. No obvious nausea, vomiting, rashes and other adverse reactions and toxic and side effects were observed in the patients in the treatment group, but the patients in the control group appeared duringthe course of treatment.Mild adverse reactions indicate that Qigui Tongluo oral liquid has high clinical safety and reliability. Conclusion:Qigui Tongluo oral liquid and Betahistine Mesylate Tablets are effective for the residual symptoms of qi-deficiency blood stasis type BPPV after successful reduction. However, the clinical efficacy of Chinese medicine Qigui Tongluo oral liquid is better than that of western medicine methotrexate betahistine tablets. No obvious adverse reactions were observed during the treatment, and the safety was reliable. It is worthy of further promotion and reference in clinic.
