Clinical Research On Curative Effect Of Scraping Therapy On Insomnia With The Heat Syndrome Due To Stagnated Liver Qi

ObjectiveThis study embark on the insomnia with heat syndrome due to stagnated liver qi,which is common in clinic.To observe the short-term and long-term effect of scraping therapy compared with acupuncture.And try to provide the basis for the study of insomnia treated by scraping.MethodsA randomized controlled clinical observation was conducted.66 patients with insomnia were randomly divided into two groups according to 1:1.The test group used scraping therapy in seven steps,namely scraping the head,scraping the cervical occiput line,scraping the Shaoyang channel in neck-shoulder,scraping the Du Meridian,scraping the Bladder Meridian on back,scraping the bilateral flanks,scraping the line between Taichong(LR 3)and Xingjian(LR 2).The therapy on head should be scraped to the scalp feel warm.The scraping on Taichong(LR 3)and Xingjian(LR 2)line should made the patients feel soreness and distension.While the other place was supposed to be scraped to the appearance of sha.The total scraping time was about 30 min.The patients taken the therapy 1 times a week.1 course contained 2 times,and 2 courses were given in the experiment.The control group used acupuncture.The point selection and operation of control group was referred to The Therapeutics of Acupuncture and Moxibustion edited by Du Yuanhao and Dong Qin.The major points were Baihui(Du 20),Yintang(EX-HN 3),Sishencong(EX-HN 1),bilateral Anmian(EX-HN 14),bilateral Shenmen(HT 7),bilateral Zhaohai(KI 6).The assistant points were bilateral Fengchi(GB 20)and bilateral Xingjian(LR 2).All points were given acupuncture therapy 2 times a week,waiting for 30 min when developed needle sensation.1 course contained 2 weeks and 2 courses were given in control group.Assessment of PSQI and the scale of TCM syndrome of stagnation of Liver-qi and Changes into Liver-fire had been made at 4 points—before treatment,1 course of treatment,2 course of treatment,4 weeks after the end of the treatment.The curative effect was evaluated 3 times after each course and 4 weeks after the treatment.During the experiment the adverse reactions were recorded in time to carry out safety assessment.Results1.66 cases of patients were included in the study.The experimental group dropped 3 people and the control group dropped 4,with the expulsion rate of 10.61%.A total of 59 cases were included in the statistical analysis,5 cases in the experimental group and 1 cases in the control group didn't finished follow-up 4 weeks after the end of the treatment,and the curative effect was evaluated only after the treatment.2.Baseline Comparison:Before the treatment,the experimental group and the control group were comparable in two groups,including age,sex,insomnia duration,PSQI total score and sub-item score,the scale for the TCM syndrome of stagnation of Liver-qi and Changes into Liver-fire.The score were in the same baseline(p>0.05).3.PSQI total score and sub-item score(Time to fall asleep,Sleep duration,Sleep quality,Sleep efficiency,Daytime dysfunction)Analysis:Intro-group comparison of the total score of PSQI and the scores of all factors(time for falling asleep,sleep time,sleep quality)and TCM syndrome scores were decreased separately in 1 course of treatment,2 course of treatment,4 weeks after the treatment.The time effect was statistically significant(p<0.05).Interaction between time and grouping was no significant difference(p > 0.05).Grouping effect was no significant difference(p >0.05).Multiple comparison in group showed that PSQI total score and sub-item scores of two groups were decreasing as courses of treatment passed on,while rebounded after 4 weeks of the treatment.Comparison between groups showed that the time to fall asleep of text group was better than control group in the first course of treatment with statistically significant(p>0.05).It showed Guasha relief difficulty falling asleep quicker.Comparison between groups after 2 courses and 4 weeks after the treatment showed that PSQI total score and sub-item scores were no significant difference(p>0.05).4.Analysis of TCM syndrome of stagnation of Liver-qi and Changes into Liver-fire:The time effect and interaction between time and grouping were statistically significant(p < 0.05).Grouping effect was no significant difference(p>0.05).Multiple comparison in group showed that TCM syndrome of stagnation of Liver-qi and Changes into Liver-fire of two groups were decreasing as courses of treatment passed on,while rebounded after 4 weeks of the treatment.Further comparison between two groups showed that the score of text group was lower than the control group with statistically significant(p<0.05).It means scraping therapy has better effect in relief TCM syndrome of stagnation of Liver-qi and Changes into Liver-fire.5.Therapeutic Evaluation:The therapeutic evaluation after 1 course,2 courses and 4 weeks after the total treatment showed no statistically significant between two groups over the rank-sum text(p>0.05).It means scraping and acupuncture has therapeutic equivalence.The total effective rate of scraping therapy group and acupuncture group were 90.0% and 79.31% after 1 course of treatment,96.7% and 96.6% after 2 courses of treatment,88.0% and 84.7% 4 weeks after the total treatment separately.There were no significant difference across Chi-square text(p>0.05).It indicated that Scraping therapy and acupuncture both had good effects in insomnia with the heat syndrome due to stagnated liver qi.There were no significant difference between two groups.ConclusionBoth Scraping and acupuncture could improve sleep quality and clinical symptoms of stagnation of Liver-qi and changes into Liver-fire.Scraping had faster effect in improving patients' fall asleep time and TCM syndrome.Since Scraping is easier to operate,fewer times of treatment than acupuncture,it can be chosen as a method of treating insomnia with the type of heat syndrome due to the stagnation of Liver-qi.It is worth generalized in clinical and family health care.
