Clincal Study On The Effect Of Intradermal Needle Therapy On Sleep Quality Of Patients With Total Hysterectomy

ObjectiveTo observe the sleep quality of the patients with total hysterectomy,and explore the therapeutic efficacy and safety of intradermal needle therapy for early post-hysterectomy patients with sleep quality.All of these is to provide a safe,effective,simple and easy treatment to raise the sleep quality of postoperative patients and accelerate the postoperative recovery.MethodsAccording to the select criteria,90 cases have been chosen for this study from those who underwent total hysterectomy with the defined surgery date in The First Affiliated Hospital of Guangzhou University of Chinese Medicine from January 2017 to October 2017.These cases were randomly divided into intradermal needle group,sham acupuncture group and blank control group with 30 cases each.Three groups were treated with postoperative routine care,rehydration therapy,antibiotic therapy,wound dressing and other basic treatments.Intradermal needle group was treated by intradermal needle therapy of the glabella,sleeping,three vaginal intercourse,foot three mile and hsin shu from the evening of the surgery to the third day after surgery.The patients in sham acupuncture received sham acupuncture therapy.The patients in blank control group received basic treatment.The Pittsburgh Sleep Quality Scale(PSQI),pain score(VAS),Self-rating Anxiety Scale(SAS),the time of the first intestinal exhaust and defecation were observed before and after the intervention.Compare difference of each observation between different intervention methods,and evaluate the intervention effectiveness.SPSS 21.0 software package was used to analyze the data collected by the institute.Results1.This study included 90 patients after total hysterectomy.During the study period,1 case in sham acupuncture group discharged in advance for personal reasons and withdrew from the study.The other patients completed the intervention and outcome of the assessment in accordance with the requirements.There were 89 cases in total,including 30 cases of intradermal needle group,29 cases of sham acupuncture group and 30 cases of blank control group.2.There were no statistically significant difference among the three groups in age,operation method,intraoperative appendix resection,use of analgesic pump before the intervention of this study(P>0.05).There were no statistically significant difference among the three groups in PSQI score,VAS score and SAS score before the intervention of this study(P>0.05).The comparison among the groups is significant.3.All the patients underwent total hysterectomy,PSQI score and its sleep factor scores were increased compared with preoperative(P<0.05).PSQI and sleep quality,sleep time,sleep disturbance and daytime functional score on the first postoperative day were the highest compared with those on the preoperative day and the third postoperative day(P<0.05).The score of sleep latency and sleep efficiency on the third postoperative day was not significantly different from the first postoperative day(P>0.05).4.The postoperative scores of PSQI,sleep quality,sleep latency,sleep time,sleep efficiency,sleep disturbance,sleep disturbance and daytime function were lower in the intradermal needle group than those measured score in the other two Group patients at the same observed time(P<0.05).5.The pain scores of three groups were highest on the first postoperative day,but the VAS scores on the first postoperative day and the third postoperative day of intradermal needle group were lower than those in the other two groups at the same observed time(P<0.05).6.The scores of anxiety and depression self-rating scale after intervention in three groups were lower than those before intervention(before surgery).SAS score of patients in intradermal needle group were lower than the other two groups(P<0.05).7.The first time of intestinal exhaust and defecation postoperatively in the intradermal needle group were shorter than the time in the other two groups(P <0.05).Conclusion1.Patients with total hysterectomy usually have poor sleep quality in the preoperative three days especially at the night of surgery.2.Intradermal needle treatment can improve the quality of postoperative sleep,and effectively reduce the time to fall asleep,prolong the sleep time,improve postoperative sleep efficiency,reduce sleep disorders as well as improve daytime function.3.Intradermal acupuncture can reduce the postoperative pain,relieve postoperative anxiety,promote postoperative recovery of gastrointestinal function,and further improve the quality of postoperative sleep.4.Intradermal needle acupuncture is a painless,safe and effective,simple,easy to be accepted by patients acupuncture therapy,which can effectively improve the quality of sleep in patients after hysterectomy.It is worth to promote the clinical application.
