Purpose: Explore the 2017 changchun university of Chinese medicine affiliated hospital department ii encephalopathy rehabilitation therapy in ischemic stroke patients the onset of cognitive impairment and the distribution of syndromes,based on demographic factors,number and positions of lesions and the course of analysis and comparison of the distribution characteristics of each syndrome combination,mastering the clinical characteristic of cognitive disorder in patients with ischemic stroke and TCM syndrome distribution.Based on the above data digging,provide the basis for clinical diagnosis and treatment of the disease,and to put forward a stroke of cognitive dysfunction after laying a foundation for the standardization of the intervention of traditional Chinese medicine research.Method: The cross-sectional study method was adopted.All patients' data were from ischemic stroke patients from January 2017 to December 2017 in the brain rehabilitation department of the second department of the affiliated hospital of changchun university of traditional Chinese medicine.Passed through the line of standard selection and assessment of neuropsychological scale,will include all of the patients with ischemic stroke on the basis of cognitive situation into three groups,namely the normal cognitive group,mild cognitive impairment and dementia group,carry on the inspection,auscultation and olfaction,inquiry,and pulse-taking and palpation,and records of TCM four diagnostic information,and fill out the factors of TCM syndrome diagnosis scale according to the requirement,the records of the patient's general information,history of smoking,drinking,imaging examination results and statistics.Excel2016 and SPSS19.0 were used to analyze relevant data in the data.The measurement data were tested by normal distribution and homogeneity of variance.Kruskal-wallis test and mann-whitney U test in nonparametric test were used for comparison of measurement data,and chi-square test was used for comparison of counting data.P < 0.05 indicates that the comparison between groups is statistically significant.Result:1.Morbidity: 427 patients were selected for stroke,among which 188 were dementia,accounting for 44.03%,and 112 people with mild cognitive impairment,accounting for26.23%,and 127 in normal cognition group,accounting for 29.74%.2.Demographic data: comparison between the gender groups: dementia group than men(63.3%)than the normal cognitive group(39.4%)(P < 0.05)and mild cognitive impairment group of female ratio(55.4%)is lower than the normal cognitive group(60.6%)(P < 0.05);Dementia is compared between age groups: groups of age group(69.02±7.32)higher than that of mild cognitive impairment(64.32±8.42)(P < 0.05),dementia age is higher than the normal cognitive group(59.56±8.22)(P < 0.05),mild cognitive impairment age is higher than the normal cognitive group(59.56±8.22)(P < 0.05);By the fixed number of year of the education comparison between groups: dementia group by education fixed number of year(9.89-4.04)less than mild cognitive impairment(10.63 ± 3.61)(P < 0.05),dementia by education life below the normal cognitive group(11.54±2.66)(P < 0.05)and mild cognitive impairment by education life below the normal cognitive group(11.54±2.66)(P < 0.05).3.Distribution of syndromes: in a single independent syndromes,appears the frequency is more obsessed with the to phlegm turbidity syndrome(63.00%),spleen and kidney deficiency syndrome(32.78%),other card type distribution in turn is Yin hand syndrome(27.87%),brain collaterals syndrome and blood stasis resistance(23.65%),toxic heat syndrome(23.42%),inside qi deficiency syndrome(22.95%),relieving ZhiZhuo leave card(18.03%);Among the complex syndromes,the two syndromes were the most common,while the other syndromes were relatively few.The two syndromes,spleen and kidney deficiency syndrome and sputum turbidity syndrome,were the most common,accounting for 24.82%.Imaging examination: bilateral lesions of stroke patients,the largest of 251 people,accounting for 58.78% of the total,the spleen and kidney deficiency,phlegm turbidity/certificate of highest incidence of cognitive impairment,81.82%,78.85%;The number of stroke patients with more than 4 lesions was the highest,232,accounting for54.33% of the total number,among which the incidence of cognitive impairment caused by deficiency of spleen and kidney,turbid phlegm and dysplasia was the highest,at 85.04% and79.59%.Conclusion:1.The incidence of cognitive impairment after stroke is high,about 70%,and age,gender,educational background and other factors are related to the occurrence of cognitive impairment after stroke.2.Patients with multiple lesions and short course of disease are more likely to suffer from cognitive impairment,and the syndromes of traditional Chinese medicine mainly include spleen and kidney deficiency and turbid sputum.3.The TCM syndromes of cognitive impairment in stroke patients mainly include deficiency of spleen and kidney and obstruction of phlegm;The syndrome of deficiency of spleen and kidney and the syndrome of turbid phlegm may be associated with cognitive impairment and even dementia.
