Objective:This study measured total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),triglyceride(TG),apolipoprotein A1(ApoA1),apolipoproteinB(ApoB)and lipoprotein(a)[Lp(a)] levels of patients with colorectal adenomas(CRA)or colorectal adenocarcinomas(CRC),and analyzed the relevance,and studied the relevance of blood lipid in different pathological types of CRAand different stages,different degrees of differentiation and lymph node metastasis of CRC patients.In addition,compared the blood lipid levels between preoperative and postoperative patients with CRA,and analyzed the causes,in order to provide new thought in the diagnosis and treatment of CRA and CRC.Method:Collected the clinical and laboratory data of patients from January 2016 to December 2017 in the First Hospital of Jilin University Gastroenterology,whose pathological types were adenoma and adenocarcinoma.According to the pathological type,patients were grouped into CRA group(n=69),CRC group(n=70),and then analyzed differences of levels of TG,TC,LDL-C,HDL-C,ApoA1,ApoB,and Lp(a).According to the pathological type,the CAR group can be divided into tubular adenoma group(n=53),the tube-villous adenoma group(n=10)and villous adenoma group(n=6),and then analyzed differences of levels of TG,TC,LDL-C,HDL-C,ApoA1,ApoB,and Lp(a).According to the TNM stages,the CRC group can be divided into group 0(n = 2),group?(n = 7),group ?(n = 18),group ?(n = 13),group ?(n = 6),and then analyzed differences of levels of TG,TC,LDL-C,HDL-C,ApoA1,ApoB,and Lp(a).Patients with CRC can be divided into preoperative group(n = 70)and then analyzed differences of levels of TG,TC,LDL-C,HDL-C,ApoA1,ApoB,and Lp(a).According to the pathological type,the CAC group can be divided into lymph node metastasis postive group(n=32)and lymph node metastatic negative group(n=38);the middle-differentiated adenocarcinoma group(n=58)and the moderatedly differentiated adenocarcinoma group(n=12),and then analyzed differences of levels of TG,TC,LDL-C,HDL-C,ApoA1,ApoB,and Lp(a).All data were statistically analyzed using SPSS24.0.P < 0.05 was considered statistically significant,and p < 0.01 prominently statistically significant.Result:1.The TC level of patients with CRA was significantly higher than that in patients with CRC,and the difference was statistically significant(P < 0.05).The LDL-C and ApoA1 level of patients with CRA was significantly higher than that in patients with CRC,and the difference was prominently statistically significant(P < 0.01).The Lp(a)level of patients with CRA was significantly lower than that in patients with CRC,and the difference was statistically significant(P < 0.05).The levels of TG,LDL-C,Apo B between patients with CRA and CRC had no statistically significant differences(P > 0.05).2.The level of TC in villous adenoma group significantly higher than the tubular adenoma group and villous-tubular adenoma group,the difference was statistically significant(P < 0.05).There was no statistically significant difference between the other three groups(P>0.05).3.According to the TNM stages,comparison of colorectal cancer patients with blood lipid,the level of TC in patients with group?below group 0,group ? below group?,group ? below group?,the differences were statistically significant(P < 0.05);the level of HDL – C,group ? lower than group 0,the differences are prominently statistically significant differences(P < 0.01),group ?below group?,group ? below group ?,differences were statistically significant(P < 0.05).There was no statistically significant difference between the other indicators(P>0.05).4.Compared the levels of blood lipid between postoperative patients and preoperative pateints,we could find that TC,LDL-C,and HDL-C levels of preoperative pateints were significantly higher than that of postoperative patients,the difference was statistically significant(P < 0.05).There was no significant difference between the two groups in other indicators(P < 0.05).5.There was no significant defference in blood lipid and lipoprotein in patients with different lymph node metastasis of CRA(P>0.05).And there was no significant difference in serum lipid and lipoprotein levels in patients with middle and low differentiation adenocarcinoma(P>0.05).Conclusion:1.TC,HDL-C,ApoA and Lp(a)maybe involved in the development of adenomas into adenocarcinomas.2.The TC is associated with the increase of villi in adenomas(or increases the probability of cancerization).3.With the development of CRC,the later stage of TNM staging,the poorer prognosis,and the overall level of TC and HDL-C showed a gradual decline.4.In the basic of removing effects of the tumor tissue,the serum cholesterol levels showed a trend of increase,then we maybe can predict the metabolism of tumor cells has the correlation with the cholesterol levels.
